CONSENT AND HEARING WAIVER FORM Pacific Gas and Electric Company, hereby agrees to comply with the terms and conditions of the Confirmatory Orders arising out of the Diablo Canyon Power Plant and Diablo Canyon Independent Spent Fuel Storage Installation submittal dated September 24, 2013, as supplemented by letters dated December 18, 2013, and May 15, 2014. By signing below, Pacific Gas and Electric Company consents to the issuance of the referenced Confirmatory Order, and by doing so, agrees to waive the right to request a hearing on all or any part of the Confirmatory Order.
