ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation (NOV) for two violations associated with a White Significance Determination Process finding will be issued on or about February 25 2015, to Entergy Nuclear Operations, Inc. (licensee) as a result of an inspection. The White finding, an issue of low to moderate safety significance, will result in additional NRC inspections.

The White finding involves the licensees failure to monitor the highest exposed part of a workers whole body during control rod drive housing replacement activities in February and March 2014 because it failed to properly assess new dose gradients caused by the use of personal shield vests.

The two violations involve the failure to comply with the requirements of 10 CFR 20.1201(c) and Technical Specification 5.4.1.a. Specifically, the licensee did not (1) properly use the deep-dose equivalent (DDE), and (2) determine the effective dose equivalent (EDEX) using a dosimetry method approved by the NRC as required by 10 CFR 20.1201(c). In addition, the licensee failed to establish a procedure for personnel monitoring covering all practical worker positions and shielding geometries as required by Technical Specification 5.4.1.a.

The NRC issued a preliminary White finding for this issue on December 2, 2014. As a result, a Regulatory Conference was held on January 13, 2015. During the meeting, the licensee agreed with the performance deficiency and the violation, but disagreed with the significance of the finding. The NRC reviewed the additional dose reassessment information presented during the conference and concluded that the finding is appropriately characterized as White with a low to moderate risk significance.

It should be noted that the licensee has not been specifically informed of the enforcement actions. The schedule of issuance and notification is:
