RESPONSE TO NOTICE OF VIOLATION AND NRC INSPECTION
REPORT 05000285/2014009
Dear Mr. Cortopassi:
Thank you for your letter, dated October 20, 2014 (ML14293A237), in which you responded to
NRC Inspection Report 05000285/2014009 and Notice of Violation, dated September 18, 2014
(ML14261A455). The violation involved the failures to:
correctly translate applicable regulatory requirements and design bases into plant
procedures regarding strong pump - weak pump interaction affecting auxiliary feedwater
pumps
correctly translate applicable regulatory requirements and design bases into plant
procedures, in that OPPD failed to ensure that raw water cooling was provided down to
the design basis low river level of 976'9" mean sea level
maintain the effectiveness of the emergency plan, in that emergency action level HA1,
Natural or destructive phenomena affecting the Protected Area, contained an
inaccurate river level
correctly translate applicable regulatory requirements and design bases into plant
procedures, in that OPPDs calculation for fuel consumption did not assume that the
diesel generators were run at 60.8 hertz, for the entire 7-day mission time
perform a written evaluation for a design change that may have required NRC review
and approval, in that OPPD did not evaluate a change that would permanently substitute
manual actions for an automatic action to add water and nitrogen gas to the component
cooling water surge tank

After reviewing the information you provided in your response to the Notice of Violation, we
have concluded that your completed and proposed actions appear to adequately address the
regulatory noncompliances described in the Notice of Violation. We will review your
implementation of these corrective actions during a future inspection to determine whether full
compliance has been restored and whether compliance will be maintained.
In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter will be
made available electronically for public inspection in the NRC Public Document Room or from
the NRCs Agencywide Documents Access Management System (ADAMS), accessible from the
NRC Web site at http://www.nrc.gov/reading-rm/adams.html.
