June 17, 2014 EN-14-023 OFFICE OF ENFORCEMENT NOTIFICATION OF SIGNIFICANT ENFORCEMENT ACTION Licensee: Entergy Operations, Inc. (EA-14-008)

ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violation will be issued on or about June 20, 2014, to Entergy Operations, Inc. (Entergy) for a violation associated with a Yellow Significance Determination Process finding for Unit 1 and a Yellow Significance Determination Process finding for Unit 2 identified during an inspection of Arkansas Nuclear One. The Yellow findings representing issues of substantial safety significance will require additional U.S.

Nuclear Regulatory Commission (NRC) inspections. The findings involve the March 31, 2013, Unit 1 stator drop that affected safety-related equipment on both units. Specifically, Entergy approved a design for the temporary hoisting assembly that was not supported by detailed drawings, specifications, evaluations, and/or certifications as required by Entergy Quality Procedure EN-MA-119, Material Handling Program. Entergy failed to identify the load deficiencies in the heavy lift gantry calculation, failed to design the temporary hoisting assembly for at least 125 percent of the projected hook load, and failed to perform a load test in all configurations for which the temporary hoisting assembly would be used. As a result, on March 31, 2013, while lifting and transferring the main generator stator, the temporary overhead crane collapsed causing the 525-ton Unit 1 stator to fall on and extensively damage portions of the plant, including safety-related equipment.

A Notice of Violation is included based on Entergys failure to meet 10 CFR Part 50, Appendix B, Criterion V, Instructions, Procedures and Drawings, which requires, in part, that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings. In accordance with the NRC Enforcement Policy, the NOV is considered an escalated enforcement action because it is associated with a Yellow finding for Unit 1 and a Yellow finding for Unit 2.

It should be noted that the licensee has not been specifically informed of the enforcement action. The schedule of issuance and notification is:
