As MRO, I am satisfied that the appropriate actions have taken to resolve the issue. If any further questions arise please do not hesitate to give me a call at 573-676-4301. Sincerely, William P. March 7, 2014 Re: 1401DIL To whom it may concern: d 7 No. 7109 P. 1 We have had several clients who have received "NEG" results from their labs for urine samples ftotn lot# 1401011. Those samples should have been resulted NEG, DILUTE". The results from our verification sample sent to Quest, Lenexa listed a creatiniine of 15.6 and a specific gravity of 1.002. Results from other labs on specimens sent AFTER those sent to you also gave 'dilute' results. Due to the inconsistancies, I retired lot 1401DIL and prepared lot 1403DIL which assays at creatinine =10.4 with a specific gravity of 1.0013. A very small deviation on the creatinine calibration solutions can easily move a 15 creatinine to a 20 creatinine, so a lab mismeasurement could be a possibility I would be happy to send you samples of the new lot if you would like to further test e qua 1tati values of our and/or your lab.
