TN0026450 - 2013 DISCHARGE MONITORING REPORT QUALITY ASSURANCE (DMR-QA) STUDY 33 PROVIDER-GRADED TEST RESULTS AND CORRECTIVE ACTION REPORT Please find enclosed the provider-graded test results of the 2013 DMR-QA Study 33 Laboratory Performance Evaluation obtained by TVA Sequoyah Nuclear Plant (SQN) and supporting laboratories as required by NPDES Permit TN0026450.

The graded result for study WP-216 for Environmental Science Corporation (ESC) was found to be 'Not Acceptable' for Non-Filterable Residue Total Suspended Solids (TSS). Reanalysis of TSS was performed in study WP-218 and found to be 'Acceptable.' Enclosed is ESC's quality control review and corrective action letter.

Juliet, TN 37122 615-758-5858 1-800-767-5859 Fax: 615-758-5859 www. envsci. com July 29, 2013 To: All DMR-QA 33 Participants Re: ESC DMR-QA 33 Chemistry Failures ESC has performed a quality review on the DMR-QA 33 PT outliers (analyzed in the WP-216 study) concerning Non-filterable Residue and Amenable Cyanide by various methods, Boron by EPA 6010B/6010C, and Cobalt/Manganese by EPA 200.7. We were unable to determine obvious causes of these failures. ESC has since analyzed another PT for these analytes with acceptable final results. We believe these are isolated incidences and have evaluated all QC criteria concerning this matter. The laboratory has performed all appropriate corrective actions and made changes where necessary.
