NORTH ANNA POWER STATION - INVESTIGATIVE SYNOPSIS, OFFICE OF INVESTIGATIONS CASE NUMBER 2-2013-016
Dear Mr. Heacock:
Enclosed for your information is the synopsis of the U.S. Nuclear Regulatory Commission (NRC) Office of Investigations (OI) completed report regarding a former security officer who was deliberately inattentive to duty while posted in a Bullet Resistant Enclosure (BRE).
Based on the results of the investigation, the NRC has concluded that the former security officer violated Title 10 of the Code of Federal Regulations (10 CFR) 50.5 by engaging in deliberate misconduct that caused Dominion Nuclear to be in violation of 10 CFR 73.55(j)(3). Specifically, the former security officer was deliberately inattentive to duty, which precluded Dominion Nuclear from ensuring that all on-duty security force personnel are capable of maintaining continuous communications with each alarm station.
The NRC acknowledges the very low security significance of this issue, Dominion Nuclears identification of the matter, and subsequent corrective actions. Regional security inspectors will review and document the enforcement aspects of this performance deficiency during the next scheduled security baseline inspection.
Dominion Nuclear is not required to respond to this letter. In accordance with 10 CFR 2.390 of the NRC's "Rules of Practice," a copy of this letter, and its Enclosure, will be available electronically for public inspection in the NRC Public Document Room, or from the Publicly CERTIFIED MAIL NUMBER: 7012 1010 0002 6821 3370 RETURN RECEIPT REQUESTED

Official Use Only - OI Investigation Information Case No. 2-2013-016 Enclosure SYNOPSIS This investigation was initiated on January 31, 2013, by the U.S. Nuclear Regulatory Commission (NRC), Office of Investigations (OI), Region II, to determine whether a former security officer/technical specialist, at the North Anna Power Station, willfully failed to be inattentive to duty.
Based upon the evidence developed during this investigation, OI Region II substantiated that the former security officer/technical specialist, at the North Anna Power Station, was deliberately inattentive to duty.
