ES-401 Record of Rejected K/As Form ES-401-4 Tier / Randomly Reason for Rejection Group Selected K/A RO EXAM 1/1 022 K1.02 After a significant effort, unable to develop an operationally valid discriminatory exam question due to no unique and credible relationship between Loss of Reactor Coolant Makeup and charging flow to pressure differential between charging and RCS. Palisades utilizes positive displacement charging pumps. Re-sampled and obtained K/A 008 AA2.06.

1/1 065 AA2.06 After a significant effort, unable to develop an operationally valid discriminatory exam question due to no unique and credible relationship between Loss of Instrument Air and knowledge of when to trip the Reactor when Instrument Air pressure is decreasing. The decision to trip the Reactor at Palisades is a judgment decision made by Operators. Re-sampled and obtained K/A 065 AA2.03.

2/1 007 G 2.4.47 After a significant effort, unable to develop an operationally valid discriminatory exam question due to no unique and credible relationship between Pressurizer Relief/Quench Tank and the ability to diagnose and recognize trends in an accurate and timely manner utilizing the appropriate control room reference material. Re-sampled and obtained K/A 007 G 2.2.39.

2/1 012 K5.01 This K/A was rejected due to similarities with Question # 28. Re-sampled and obtained K/A 012 K5.02.

2/1 073 K5.03 After a significant effort, unable to develop an operationally valid discriminatory exam question due to no unique and credible relationship between Process Radiation Monitoring and knowledge of the operational implications as they apply to concepts of the relationship between radiation intensity and exposure limits. Re-sampled and obtained K/A 073 K4.01.

2/1 103 A4.06 After a significant effort, unable to develop an operationally valid discriminatory exam question due to no unique and credible relationship between the ability to manually operate and/or monitor in the control room with regards to operation of the containment personnel airlock door. There is no method to monitor or operate the containment air lock door from the control room at Palisades. Re-sampled and obtained K/A 103 A4.04.

2/2 033 A3.01 After a significant effort, unable to develop an operationally valid discriminatory exam question due to no unique and credible relationship between Spent Fuel Pool Cooling and the ability to monitor automatic operation of the Spent Fuel Pool Cooling System including:

Temperature control valves. There are no automatic temperature control valves for the Palisades Spent Fuel Pool Cooling System. Re-sampled and obtained K/A 033 A3.02.
