1. Pg. 24, Item #2 under This review determined the following, the point that the applicant could have also discussed valve location in order to end the casualty really needs to be driven home.

The facility response that the OATC should not be burdened (emphasis mine) with the additional responsibility of considering and recommending actions related to a problem or failure is WRONG! They state that focus will be removed from monitoring core reactivity. We expect that all crew members will stay engaged during a problem/casualty and provide the necessary input to mitigate any such situation as necessary. This is not an adequate defense for non-action!
