I also need a complete copy of the applicants individual examination report, to include all ES-303-1 and 2 Forms (cover page, walk-through grading details page [page 2], SRO simulator operating test grading details [page 3b], and all operating test comments pages). I didnt get this while visiting Region II because I didnt want to carry around PII.
If you have an electronic version, that would be fine. (I currently have most of the applicants grade report electronically, but not all of it.) Otherwise you can use internal NRC mail in a double envelope to David Muller, Headquarters at Mail Stop O7G-13. Of course, you could have your OLA take care of this if you are pressed for time.
