NRC FFD Program Performance Data Reporting System 10 CFR Part 26, Subpart I - Managing Fatigue Attachment This attachment provides the entire narrative as the NRC form has been created with boxes that scroll down, and when printed, the information does not print in its entirety due to the way the NRC has built the electronic FM form.
Analysis of Waiver Assessment Data:
No waivers were initiated at Wolf Creek for 2012.
Analysis of Fatigue Assessment Data:
One for-cause fatigue assessment was completed from 01-01-2012 through 12-31-2012. Wolf Creek had zero fatigue assessments completed for a waiver or for a self-declare of fatigue. One worker did self-declare during 2012 and was sent home for a minimum of a 10-hour break and no fatigue assessment was performed or required.

Conclusions:==
Wolf Creek initiated zero waivers for 2012. One fatigue assessment was performed and the actions taken were found to be adequate in sending the worker home pending fitness-for-duty results. The 10-hour minimum break requirement was complied with for the for-cause and for the self-declare. No program or performance issues were noted from the collective review of the data.
Summary and Status of Corrective Actions:
The evaluation review period is 01-01-2012 through 12-31-2012. During this review period no waivers were performed, one fatigue assessment was performed, and one worker self-declared fatigue. Both the fatigue assessment and self-declare were documented in the corrective action program. In a collective review of the 2012 fatigue assessment and self-declare data, no gaps to performance or program improvement opportunities were found.
