Additional space is provided at the end of this checklist for documenting other comments.
- 1. Does anchorage of equipment in the area appear to be free of potentially adverse seismic conditions (if visible without necessarily opening cabinets)?
All nearby equipment adequately anchored Y
NEI ULI N/AEl
- 2. Does anchorage of equipment in the area appear to be free of significant Y4 NEI UEi N/AEl degraded conditions?
No degradation of anchors observed in the area.
- 3. Based on a visual inspection from the floor, do the cable/conduit raceways and HVAC ducting appear to be free of potentially adverse seismic conditions (e.g., condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?
Cable trays and conduit are too far away. Asset is protected from above by a concrete ceiling. (See Picture 6).
YONEI Uo N/Arl
- 4. Does it appear that the area is free of potentially adverse seismic spatial YX NEI UE-N/AEl interactions with other equipment in the area (e.g., ceiling tiles and lighting)?
Asset is not close enough to other equipment to have seismic spatial interactions.
'If the room in which the SWEL item is located is very large (e.g., Turbine Hall), the area selected should be described. This selected area should be based on judgment, e.g., on the order of about 35 feet from the SWEL item.

- 5. Does it appear that the area is free of potentially adverse seismic interactions that could cause flooding or spray in the area?
The sprinkler system nearby has adequate anchorage, is free of brittle attachments and has no potential interactions that would cause it to rupture.

interactions that could cause a fire in the area?
No fire hazards nearby.
- 7. Does it appear that the area is free of potentially adverse seismic interactions associated with housekeeping practices, storage of portable equipment, and temporary installations (e.g., scaffolding, lead shielding)?
There are currently workers in the area - nothing is stored improperly.

- 1. Does anchorage of equipment in the area appear to be free of potentially YN NEI U-- N/AU adverse seismic conditions (if visible without necessarily opening cabinets)?
Anchorage is robust and acceptable. Wall mounted cabinets and equipment are supported by unistrut strips anchored to the wall.
- 2. Does anchorage of equipment in the area appear to be free of significant YEI NEI UU-N/AUl degraded conditions?
Where welds are outside cabinets and visible, they are free of degradation.
- 3. Based on a visual inspection from the floor, do the cable/conduit raceways and HVAC ducting appear to be free of potentially adverse seismic conditions (e.g., condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?
Overhead cable trays have adequate seismic clearances. HVAC duct and tray supports are all robust. Trays have longitudinal bracing.
Seismic clearance between asset R1400SOOC and IYAC duct on west side of asset is 4". Per Dwg. M-5433, Rev. B (posting TCEDP 26116, Date 06/94), a minimum clearance of 6" is required (see Photo DSCN 0319). Initiated CARD 12-27959.
- 4. Does it appear that the area is free of potentially adverse seismic spatial interactions with other equipment in the area (e.g., ceiling tiles and lighting)?
No ceiling tiles in the area. With one exception, lights are supported by cables and chains. Lightfixture to and south of asset R1400SOO1C has one chain detached (see Photo DSCN 0323). Fixture is resting against stiffener on nearby HVAC duct. Cable is suppdrfing frxture where chain is detached. CARD 12-27093 was initiated to correct condition, YE] NEI UQ N/AU Yio NO UU N/AU]

Housekeeping: There are several miscellaneous sign stands placed near the wall at column lines A-12
& 13 that do not have any signs attached.
Technical evaluation TE-A30-08-066 (CARD 08-24488) shows that RP stanchions will not overturn during seismic event. These stanchions are very similar to the sign stands mentioned Therefore, there is no restriction from a seismic standpoint on their use in the plant TMPE-12-0294 Appendix D Page D-94

- 1. Does anchorage of equipment in the area appear to be free of potentially adverse seismic conditions (if visible without necessarily opening cabinets)?
Hose reel support is missing a nut, however it is not close enough to E5150F045 or E2101COO1C to pose any realistic hazard. (See Pictures 19 and 20.) CARD 12.-26957 has been written (8/21/12)for the missing nut on the hose reel support baseplate. The other equipment in the area has adequate anchorage.

The RP basket stanchion is placed in the middle of the room and is being used to delineate the contaminated area. Technical Evaluation TE-A30-08-066 shows that the stanchions will not overturn during a seismic event, therefore, there is no restriction on their use in the plant.
