CONDITION REPORT I CR-IP3-2011-02393 Originator Phone: 5520 Operability Required: N Reportability Required: N Initiated Date: 04/1912011 12:13 During the 3 month PM inspection of 31, 31A, and 3lB manholes under work orders 52316417, 52316418, and 52316412 respectively, the following was noted: All three manholes were filled to the top with water. 31 Manhole does not have any broken supports, but there appears to be cable lying in the bottom of the hole. It also looks like some of the cable has been cut off. 31A Manhole has a support that appears to have broken and is not supporting the cables in the center of the pit. 3lB Manhole appears to have all the supports intact, but they are heavily corroded. There appears to be a piece of channel lying in the bottom of the pit. Photos were taken and placed on the R Drive in the Manhole 31A Photos folder. There are previous photos in that folder depicting the same discrepancies as noted in this CR.

Per CAA review, acknowledge the CR owner (pC Eng Components) recommendation and approval to close the CR. Subresponse :

There has been previous documentations of the noted conditions in the manholes as found in past PMs. The responses to the past CRs include some repairs to the existing supports and pumping out of water. However, dewatering PM frequencies were never adjusted and the degraded supports have not been replaced. In the current conditions, there is no challenge to cable integrity since the cable is in supported condition. The following corrective actions have been performed 1 are recommended to achieve acceptable conditions:

1. Dewatering PM frequencies have been adjusted to accommodate water levels according to history:

Manhole 31 PM frequency adjusted to 1 month Manhole 31A PM frequency adjusted to 2 month Manhole 3lB PM frequency adjusted to 2 month

2. The following work orders have been generated to correct 1 repair degraded conditions found in the manholes:
