The space below each of the following questions may be used to record the results of judgments and findings. Additional space is provided at the end of this checklist for documenting other comments.

1. Does anchorage of equipment in the area appear to be free of potentially Yes adverse seismic conditions (if visible without necessarily opening cabinets)?
2. Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?

Observed that LPSI 21 is missing an anchorbolt. This condition has been evaluated by calculation CA02066.

3. Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?

Cable trays are supported on walls and trapeze hangers, judged not to have any seismic concerns.

4. Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?
5. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?

The overhead threaded fire piping is judged to be well supported and not a concern for spray or flooding.

6. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?
7. Does it appear that the area is free of potentially adverse seismic interactions Yes associated with housekeeping practices, storage of portable equipment, and temporary installations (e.g., scaffolding, lead shielding)?

The space below each of the following questions may be used to record the results of judgments and findings. Additional space is provided at the end of this checklist for documenting other comments.

1. Does anchorage of equipment in the area appear to be free of potentially Yes adverse seismic conditions (if visible without necessarily opening cabinets)?

Observed panels mounted to masonry block wall. Masonry block walls, identified as T, S, N, AA and BB on drawing A-260, are safety-relatedand thus seismically ratedper IE Bulletin 80-11.

2. Does anchorage of equipment in the area appear to be free of significant Yes degraded conditions?
3. Based on a visual inspection from the floor, do the cable/conduit raceways and Yes HVAC ducting appear to be free of potentially adverse seismic conditions (e.g.,

condition of supports is adequate and fill conditions of cable trays appear to be inside acceptable limits)?

There are open ladder type cable trays, supportedon eitherunistrut frames or a combination of unistrut and rod hangerframes or cantileverbracket off unistrutframes. Typically the cable tray is clipped to every 2nd or 3rd cantileversupport bracket.

4. Does it appear that the area is free of potentially adverse seismic spatial Yes interactions with other equipment in the area (e.g., ceiling tiles and lighting)?

Verify whether cabinets 2D02 and 2D14 are bolted together. Conduit from 2D12 is a 1/8"to 2Y10-80 & 2D02 75-2208. 1/2"gap to 2YOIA and 1/4"gap to 2Y1 1 to Block wall.

Evaluated using 8 Hz frequency for cabinets 2% Damping Ratio, 1.5 PF Response Spectrum ABO27D2N.dat A=0.25g D= 2 * (1.5*0.25*386) / (8x2x3.14)A2 = 0. 11" < 1/4" OK

Found a screw driver on floor underneath the charger, although not good housekeeping practices, this is not a seismic concern.

8. Have you looked for and found no other seismic conditions that could Yes adversely affect the safety functions of the equipment in the area?

Unistrut frames support open laddertype cable trays and some trays are supportedon cantileverbrackets with hook type end but no clips. Unistrut frames are also braced either knee bracing to the ceiling or back to the wall.

Also, there are some cable trays supportedon cantileverbrackets that are braced back to the walls.

Evaluation:

Assume each cabinet weight is equivalent to an MCC.

Per GIP Rev. 2A, Table C. 1-1, 800 lbs/cabinet, Total Weight = 1600 lbs Cabinetfrequency is estimated at 8 Hz, Response Spectrum AB027D2N.dat, Sa.cabinet = 0.25 g A-37

5. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause flooding or spray in the area?
6. Does it appear that the area is free of potentially adverse seismic interactions Yes that could cause a fire in the area?
