If you rent the property on which your water use occurs, please specify the owner's name and address. (If more than one owner please write name and address on back of form). Measurement:

Please indicate whether your source of water is surface, groundwater, or both, and specify in gallons how much water was used during the year. Also specify the maximum rate of flow (in gallons per minute} and the method of measurement.

QUESTION NO. 3 DOES NOT APPLY TO IRRIGATORS.

Note to Irrigators:

There are several methods used to calculate the amount of water used: 1. Multiply the operating capacity of the pump and irrigation system (in gallons per minute} by the number of hours the system is run and multiply the result by 60 to obtain gallons per day. Example: During July, an irrigation system using 700 gallons per minute is run for 170 hours0.00197 days <br />0.0472 hours <br />2.810847e-4 weeks <br />6.4685e-5 months <br />. Calculate:

700 x 170 x 60 = 7,140,000 gallons used during July. 2. Another common way to calculate usage is to multiply the number of acres irrigated by the number of inches of water applied and divide by 12 to obtain the number of acre-feet of water applied. One acre-foot is approximately 326,000 gallons. Example: During August, 4!J inches of water are applied to 31 acres of land. Calculate:

31 x 4:-t-:-12 = 11.63 acre-teet x 326,000 gallons= 3,791,380 gallons used in August. Water Use: Please list the date or approximate date when your water use started. For instance, if you first Irrigated in 1969 that Is when your water use began. In question No. 2 indicate the purpose(s) for which water is being used and the amount of water for each purpose. THE SUM OF THESE SHOULD BE EQUAL TO THE TOTAL AMOUNT OF WATER PUMPED OR DIVERTED (LISTED IN QUESTION NO. 2 UNDER MEASUREMENn.

A. Domestic Is defined as household purposes and subsistence, livestock watering, and Irrigation of gardens and orchards not exceeding 2.5 acres in size. B. Municipal is defined as serving a community, subdivision, rural water district, a trailer court, etc., for public consumption.

C. Irrigation is defined as water needed to supplement plant growth on land more than 2.5 acres in size. D. Recreational use is defined as water used pleasurably for purposes of swimming, boating, game fishing, aesthetics, etc. E. Industrial uses are any use of water for the purposes of producing a marketable product from mining, manufacturing, commercial poultry and livestock feedlot operations, etc. Included are uses for which water is injected into the ground, as for waste disposal, hydrocarbon displacement, etc. F. Electrical generation uses of water include hydropower dams, free-flow turbines, thennal or nuclear generation, pumped storage operations, pollution abatement, or other associated needs for producing electric energy. G. Fish and wildlife uses are those which require water for the maintenance and subsistence of fish and wildlife habitat. This includes aquaculture.

H. Drainage, dewatering, and effluent discharge refer to operations necessary to evacuate mines or quarries of water, drainage of agricultural areas, and waste disposal.
