Subject:
SB Root Cause Effort Crack Team, I spoke with Kevin Browning (RCT-Lead) and requested additional site procedures related to root cause that we did not initially receive. Also, he confirmed that the licensee does not consider the SB cracking to be a significant condition adverse to quality because it did not impair the function of the shield building. Instead it is categorized as an AR-Therefore, per our regulations, the licensee's proposed corrective actions do not have to prevent recurrence. This should generate some interest internally. Just to remind folks that I expect each of us to become' very familiar with the licensee's root cause related procedures.
M Definition of SCAQ From their CR procedure NOP-LP-2001:
Significant Condition Adverse to Quality (SCAQ) - Either an Adverse Condition, such as a failure, malfunction, deficiency, deviation, defect or non-conformance, or an adverse trend, which has resulted in or could result in a significant degradation of or significant challenge to nuclear, plant, or public safety. See Attachment 1 for examples.
