Description:

On 03-30-12, the simulator IT group was notified of a condition that exists at Clinton Power Station.

Historical information for a training scenario was found while using the Simulator Plant Process Computer (PPC). It was determined that the historical data in the Simulator PPC was not being erased when Initial Conditions were reset, the file was being appended.

Quad Cities IT SAM personnel notified the Quad Cities OTM of the possible condition existing with QC simulator PPC.

Quad Cities simulator IT and an Operations instructor were dispatched to verify the configuration of Quad Cities simulator PPC. They found that the PPC SOE (Sequence of Events) message log file does not get cleared out upon IC reset The SOE message file continues to accumulate information, up to its size limit, and then it overwrites old data.

A similar condition exists at other MW Exelon plants.

Immediate actions taken: 1. The Quad Cities Training Director and Corporate Operations Training were notified immediately after the condition was confirmed at QCNPS.

2. QCNPS has removed the file that contained historical data from prior scenarios that dated back to 02 06-2012.
3. A procedure change has been developed by Quad Cities Operations Training and Simulator IT group for steps to manually perform this action of deleting files. File removal method developed has been tested successfully. These changes are needed until an automatic removal method is developed and installed.

QCNPS White Paper on Exam Security Concerns wrt PPC SOE

Condition

Description:

Clinton simulator recently found (11 :53 AM on Friday, 3/30112) that "the PPC SOE (Sequence of Events) message log file docs not get cleared out upon IC reset, .... The SOE message file continues to accumulate information, up to its size limit, then it overwrites old data."

The issue is that, normally, an IC reset is expected to clear out all old data. If not, after an exam scenario, old data could be accessed in the system by going to message retrieval and looking for older data. This has exam security ramifications that must be explored.

Actions Taken:

1. Clinton Simulator BSC Guy notified other BSC personnel (including Willie Hemm at QCNPS) via e-mail at 1153 AM, 3/30112.
2. Willie Hemm notifies QC Sim Coordinator and IT SAM Guy (Lechmaier and Baker) by forwarding message at 11 :58 AM, 3/30112.
3. OTM at QCNPS notified of issue that could impact exam security at 1:03 PM, 3/30112. Condition verified to exist at QCNPS.
4. Notifications escalate. Clinton had notified Snook. Pannell notifies Quick.

Pannell sends e-mail to Moser who is in Ops Performance Conference with Cantera and Kennett Square which also involves Ludlam and Coon.

5. Fleet Conference calls abound on the evening of Friday, 3/30/12.
6. Clinton documented the issue in IR 1348127 in which they stated that they assessed the issue for exam compromise and determined none occurred. This determination was based on the fact that the discovering operator had already had his LORT OBE for the week and therefore didn't see anything "new". However, it did not necessarily look at whether other operators were able to somehow view data for their upcoming OBE by looking backward one week, or what if anything the data could even tell them. Dresden has also documented the issue in IR 1348182.

How Can Data be Viewed?:

The data can be viewed through the use ofRTViewer from any of the PCs on the RO/SRO desks in the simulator at MW plants only. RTViewer allows LAN access to the PPC to view plant system graphics, computer points, sequence-of-events (SOE) logs, etc.

When in the simulator, it allows viewing simulator PPC data. The potential exists for remote access, but this requires special software and access rights.

The SOE data is a collection of digital points that change state with a word description included. For example, it might read "RPOOIA: OFF - RPS DIV 1 'A' SCRAM RELAY", telling you half ofRPS may have received a scram signal. It DOES NOT show live or static numerical values such as power, pressure, level, flow, etc.

RTViewer was newly installed ~1 month ago on the PCs ofthe RO/SRO desks of the Clinton simulator. At QCNPS, it has been installed since the PPC upgrade 3 to 4 years ago.

ON Friday, 3/30/12, an operator in the Clinton simulator was demonstrating the capabilities of the system at the conclusion of a LORT training scenario when he discovered that he could go back several days and read SOE infonnation.

How to Get a PPC SOE Report:

1. Starting at the "Main Menu" screen:
2. Click the "System Administration" button.
3. Click the "Message Retrieval" button.
4. Click on line "Primary SOE Messages" to select it.

7. A PDF SOE report opens.

What Can be Inferred From Viewing the Files?

Based on a review of a copy of the SOE log from the QDC simulator PPC system as viewed in RTViewer, the Corporate Training assessment is:

  • The log provided enough information to determine that in one time period a turbine/generator trip, scram and electrical ATWS occurred, followed by SBLC injection, lowered RPV level, etc.
  • Numerical values of parameters are not provided.
  • No information is provided as to who is in the Simulator or what the scenarios are or why they are run.

To access this data, there is a deliberate series of commands that must be used to get to view the file. Unintentional compromise is unlikely to occur.

The potential exists for exam compromise should someone access historical aspects of this data and attempt to decipher with several other pieces of information that would have to be acquired elsewhere. Reconstruction would require technical and diagnostic skills applied to a deliberate effort.

As stated in TQ-AA-201, Examination Security and Administration, 2

"The actions in this procedure are intended to prevent inadvertent compromise, but not willful compromise of examination materials."

Assessment of Impact on ILT NRC Exam Development:

There is NO indication of Exam Compromise at this time.

The files are not readily viewable and require interpretation. ILT candidate access to said information is limited to time in the Simulator. During this time an instructor (or two) is normally present. No suspicious activity by candidates has been reported.

The chances can be further mitigated by implementing actions to remove the data from the Simulator PPCs following exam development activities, validations, and administration.

This will be discussed with the NRC ILT Lead Examiner on Monday, 4/2112, prior to commencement of validation activities and the final resolution documented in the WGE for the station IR #1348733.

Solutions:

IT personnel with oversight of fleet PPC systems have provided a means to delete the data while they work on a longer term fix.

The steps to erase data after reset have been drafted into TQ-QC-201-0113, Simulator Examination Security Actions Checklist. Procedure change will be processed.

ATTACHMENT 2 Event I Issues Report Format Title - Midwest Simulator Plant Process Computers Retain Historical Data Note: Most verbal reports should take no longer than one minute to compete.

1. Briefly Describe the EventJlssue that is Being Reported:

The intent is to describe with as few words as necessary. Avoid pre or post event information that is not directly related to the breakdown in defenses I barriers that contributed to the event. Avoid equipment EPN's (system and component noun names are sufficient.

IR# 1348127 On March 30 th at approximately 11 :00 a Clinton licensed operator accessing historical information from the Simulator Plant Process Computer (PPC) during a simulator training scenario identified that the sequence of events from previous scenarios could be reconstructed. It was determined that the historical data in the Simulator Plant Process Computer was not being deleted when the simulator Initial Conditions are reset.

The operator who discovered this condition had already taken the cycle's Out of-the-Box Evaluation (OBE) so there was no inadvertent compromise of exam material as a result of this discovery. Clinton Training personnel reviewed TQ-AA-201, the exam administration procedure, and determined that no exam compromise had occurred.

  • Emergency response (Le., First Aid, Rescue, Haz Mat, Security, Ambulance, Fire Department, etc.).
  • Stopped work & placed equipment in a safe condition

  • Interim action(s) to prevent recurrence (including planned duration)

The simulator IT group and fleet simulator peer group were informed and a method was implemented that deletes the historical data from the Simulator PPC. This was completed at Clinton and the method was shared with each site's simulator coordinator.

Conducted fleet Training Director calls with corporate IT to develop immediate actions and initiate an extent of condition review.

3. Extent of Condition Site and/or fleet, if known.

Each of the Midwest sites' simulators is affected by this interface with the simulator PPC. The Mid-Atlantic sites have not completed the simulator PPC Prompt Investigation Page 1

upgrade and do not have the ability to access historical data from the simulator "at the controls" area.

Two Midwest sites (Dresden and Quad Cities) are in the process of developing regulatory exams. These sites completed a reset of the simulator PPC historical data. Additionally they determined that historical PPC computer points could be accessed from the simulator :at the controls" area however doing so would require a deliberate series of commands and specific knowledge of when exam material was being created and validated.

Accessing this information would require a willful act on the part of the student. It is unlikely that a student would inadvertently access the information and conclude it was part of an exam.

The remaining three Midwest sites (Braidwood, Byron, and LaSalle) are not developing exams and will complete the manual reset actions this morning prior to the start of training.
