Updated 6/11/12 WORKSHOP #5, Issue Screening Davis-Besse Loss of Feedwater Event Your TEAM assignment Part 1) The following observations from the Davis-Besse Loss of Feedwater Event (Workshop #4) have been determined to be Issues of Concern (IOCs):

  • Failure to initiate feed and bleed cooling
  • Operating with unresolved main feed pump problems For each of the IOCs, answer the following questions using MC 0612 and its appendices.
  • Is the issue a performance deficiency?
  • Is the issue a finding?
  • Is the issue a violation? If so, is significance to be determined by traditional enforcement methods or via the ROP?

Part 2) Using MC 0310 and MC 0612 and its appendices determine the following:

  • Which cross-cutting area(s) is/are affected by the finding?
  • For each cross-cutting area, which component(s) affect the finding?
  • For each cross-cutting area component, which aspect(s) contribute most to the cause of the finding?
  • How does the finding meet the requirements for documentation described in MC 0612 (eg, URI, NCV, NOV, )?

Prepare a presentation of your results and be ready to discuss the technical logic of your decisions.
