Dear Mr. Ventosa:
The Region I Field Office, NRC Office of Investigations (OI), initiated an investigation (Case No.
1-2011-022) on February 17, 2011, to determine whether an Instrumentation & Controls (I&C)
Supervisor forged signatures and other information on a Technical Specification required procedure for the calibration of temperature instruments on the 23 emergency diesel generator (EDG). Based on the results of the investigation, the NRC found insufficient evidence to conclude that there was any willfulness on the part of the I&C Supervisor.
Nevertheless, a violation for failing to follow procedures was identified. This issue was entered into the Corrective Action Program by Entergy personnel and corrective actions were initiated.
The safety significance of the violation was evaluated by the NRC and was found to be minor because the NRC did not identify any instances where the required work was not properly performed. Minor violations represent items of very low safety significance and are not subject to formal enforcement action or documentation by the NRC. Therefore, this minor violation will not be documented in an inspection report and no further regulatory action is planned.
