PALISADES NUCLEAR PLANT - NOTIFICATION OF AN NRC SUPPLEMENTAL INSPECTION AND INFORMATION REQUEST

Dear Mr. Vitale:

By letter dated January 3, 2012 (ADAMS ML120030406), the NRC communicated to you the final significance determination for a finding having low-to-moderate safety significance (White) at your Palisades Nuclear Plant. The finding documented that the turbine-driven auxiliary feedwater pump was unable to fulfill its safety function for 87 days from the time of the last successful surveillance test until repairs were made to the pump turbine on May 11, 2011, and that this failure was caused by improper maintenance performed on the turbine during the 2010 refueling outage due to both an inadequate maintenance procedure and failure to follow the maintenance procedure.

By letter dated May 2, 2012, we were notified by your staff that you were ready for the NRC to conduct our supplemental inspection to review the actions taken to address the White finding.

Accordingly, we plan to conduct a supplemental inspection, beginning on June 25, 2012, at the Palisades Nuclear Plant using Inspection Procedure 95001, Supplemental Inspection for One or Two White Inputs in a Strategic Performance Area. This on-site inspection is scheduled to be performed June 25 through June 29, 2012, and the inspection team will be led by Jason Draper, a Reactor Engineer in the NRC Region III office. The results of this inspection will be documented in Inspection Report 2012010. In order to minimize the impact to your on-site resources and to ensure a productive inspection for both organizations, we have enclosed a request for documents needed for this inspection. The documents identified are those that the inspectors will need prior to and upon arrival at the site. It is important that all of these documents are up-to-date and complete in order to minimize the number of additional documents requested during the on-site portion of the inspection.

We have discussed the schedule for these inspection activities with your staff and understand that our regulatory contact for this inspection will be Mr. Michael Sicard of your organization. SUPPLEMENTAL INSPECTION DOCUMENT REQUEST Please assemble the following documents, preferably on a CD-ROM, and send to the lead inspector, Jason Draper, at the NRC Region III office. The information sent should be current as of the date of your mailing. Information provided on-site should be the latest approved documentation. The information should be mailed in such a manner as to reach the addressee by June 11, 2012. Proprietary information should be clearly marked and stored on a separate CD-ROM. Draft documentation should not be provided as part of this information request.

Administrative procedure(s) governing the sites corrective action program; Administrative procedure(s) governing the identification and resolution of problems; Administrative procedure(s) governing root cause and apparent cause evaluations; Administrative procedure(s) governing common cause assessments, effectiveness reviews, and self-assessments; Administrative procedure(s) governing the use of vendor documents and recommendations; A copy of all revisions since October 6, 2010, of procedure FWS-M-6, Auxiliary Feedwater Turbine Maintenance; A copy of Work Order 214219, including worker comments/completion notes.

Reports:

The root cause investigation report(s) for the White finding related to the turbine-driven auxiliary feedwater pump failure; A copy of all documents referenced in the root cause report for the White finding; A copy of any corrective action documents related to the root cause investigation report; A list of all condition reports associated with the turbine-driven auxiliary feedwater pump since October 2010, including a title or brief description, organized by date; A copy of any Quality Assurance or Nuclear Oversight assessments of the root cause investigation for the turbine-driven auxiliary feedwater pump failure; Documentation supporting completion of any corrective actions for the turbine-driven auxiliary feedwater pump failure; Current schedule and resource commitment documents for open corrective actions associated with the turbine-driven auxiliary feedwater pump; Schedule and status of any ongoing, planned or completed effectiveness reviews stemming from the root cause evaluation for the White finding.

Also, please have the following documents available for the inspectors upon their arrival on-site:

Updated Final Safety Analysis Report; Technical Specifications; Administrative Procedures; Site Organization Charts; Drawings; Any previously provided documents that have been revised.
