Dear Mr. Miller:

This letter is a follow up to the verbal notification made to DEQ by Dominion on Wednesday, February 15, 2012 at approximately 12:42 PM to report an overflow of raw sewage water to a stormwater ditch that discharges to Lake Aima.

On February 14, 2012, at approximately 12:50 PM, North Anna Power Stations chemistry staff was notified of water drainage which appeared to be originating in the vicinity of a manhole cover presumed to be part of the stormwater conveyance, and draining into a nearby stonnwater ditch. This water was believed to be stormwater runoff from the area surrounding the Training Building. The quality of the water appeared clear with no obvious indicators such as odor or solids. At about the same time the flow from the manhole was observed, station staff discovered that a circuit breaker inside the Training Building had been tripped which supplies power to the buildings sewage lift station. The power was restored to the Training Buildings sewage lift station at approximately 2:15 PM and the water levels in the sewage lift station tank returned to normal.

Upon further investigation on February 15, 2012 at approximately 10:30 AM, it was discovered that the design of Training Buildings sewage sump includes a line that leads to the manhole where the water flow was observed. The lift stations high level alarm was never triggered because of the power loss. The high level in the lift station resulted in a backflow to the manhole. It is estimated that approximately 120 to 200 gallons of raw sewage water could have entered the stormwater ditch. The ditch travels approximately 180 feet before entering Lake Anna. Some of the sewage water was absorbed by the ground surrounding the manhole.

However, the total volume of sewage water that reached the stormwater ditch and Lake Anna is not known.

Mr. Miller 2/20/12 Page 2 In the interim, the chemistry staff is inspecting the Training Buildings lift station once per shift to ensure it has power. A condition report (CR) was submitted to document the event.

An evaluation of the design adequacy of the Training Buildings sewage lift station is in progress. Corrective actions will be implemented based on the results of this evaluation.
