Reply to Notice of Violation IA-11-043 Dear Mr. Regional Administrator REGION IV I received the correspondence for Notice of Violation (NOV) IA-11-043. The letter was for misuse of computer for internet use at ATC area in Main Control Room. The NOV required that I respond in writing to the information it contained. This letter provides my response.
First, and foremost, accept full responsibility for my actions and the cited violation. The violation was the result of a lack of judgment on my part regarding the significance of the procedural restriction on internet usage in the control room. Please understand that I take my license and the reliance the NRC, my co-workers, and the public place upon me very seriously.
This event has provided a sober reminder of my responsibility to perform my job with the utmost diligence at all times, including strict adherence to m!. applicable procedures.
Second, even though I know that I will perform my job in accordance with all applicable requirements, as described below, both the company and I have taken concrete corrective actions associated with this event. I have learned from this experience and I understand the seriousness of my actions and my responsibility to comply with procedures consistent with my reactor operator's license and the regulations. I will ensure that my actions as a licensed reactor operator reflect my responsibility to place nuclear safety at the forefront of my decisions and actions.
Since the incident, I and Entergy have taken steps to correct this issue and avoid further violations:
I received discipline for my conduct, consisting of a disciplinary letter in my permanent plant record.
Entergy developed and implemented a new corporate procedure, EN~FAP-IT-002, designed to block internet access to select machines.
I reviewed EN*OP-llS*02 to ensure I am aware of appropriate control room conduct and access controls.
I reviewed other Entergy Corporation administrative procedures so I can be aware and understand all requirements and expectations of my license.
I recently had a face to face meeting with the Vice president of the River Bend Station to discuss how the violation impacted the company and nuclear industry as a whole.
I have reinforced, and continue to reinforce with peers and subordinates the importance of strict procedure adherence as it applies to Administrative Procedures and Policies, in addition to System Operating Procedures.
As of this date, full compliance has been achieved.
