Pursuant to the Licensing Board's Memorandum and Order dated September 15, 2010, the Staff will monthly provide the parties an index identifying the hearing file and mandatory disclosures in accordance with 10 C.F.R. §§ 2.1203 and 2.336. For purposes of producing the mandatory disclosures and hearing file, and considering any document as privileged or protected, the Staff treated documents created by agents and contractors of the NRC the same as if those documents were created by the Staff. Accordingly, the Staff produced only one document for disclosure, ADAMS Accession No. ML11334A144, Submittal DCL-11-122, Revision to the Unit 1 Reactor Vessel Material Surveillance Program Withdrawal Schedule dated November 22, 2011. Attached is the Affidavit of Leslie T. Perkins, as required by 10 C.F.R. § 2.336(c).

AFFIDAVIT OF LESLIE T. PERKINS CONCERNING MANDATORY DISCLOSURE REQUIREMENT OF 10 C.F.R. § 2.336(bl I, Leslie T. Perkins, do hereby state as follows:

I am employed as a Project Manager in the Division of License Renewal in the U.S. Nuclear Regulatory Commission's ("NRC") Office of Nuclear Reactor Regulation. I serve as a project manager for the Diablo Canyon Nuclear Power Plant, Units 1 and 2 license renewal application ("LRA"). Among other things, I am managing the preparation of the hearing file and mandatory disclosures in connection with the LRA at issue in this proceeding.

I hereby certify that all relevant materials related to the issues in connection with the LRA that are required to be disclosed pursuant to 10 C.F.R. § 2.336 (b) and (c) in the captioned proceeding have been disclosed, and that the disclosures are accurate and complete for documents identified by the Staff as of December 15, 2011.
