Exelon Generation Company, LLC.

EA-11-221 Limerick Generating Station, Unit 2 Docket No. 50-353

Subject:

ISSUANCE OF FINAL SIGNIFICANCE DETERMINATION AND NOTICE OF VIOLATION This is to inform the Commission that a Notice of Violations associated with a White Significance Determination Process finding will be issued on or about December 8, 2011, to Exelon Generation Company, LLC. (Licensee), as a result of inspections at Limerick Generating Station, Unit 2. This White finding, an issue of low to moderate safety significance that will require additional U.S.

Nuclear Regulatory Commission inspections, involved the failure to ensure sufficient technical guidance was contained in operating procedures regarding securing feedwater (FW) long-path flushing. This resulted in two FW valves (one of which was also a primary containment isolation valve (PCIV)) failing to fully shut, rendering the PCIV inoperable for greater than the Technical Specification allowed outage time. Additionally, these valves (not in the fully shut position) altered the reactor core isolation cooling flow path and rendered the system inoperable for greater than the Technical Specification allowed outage time.

The licensee was offered the opportunity to attend a Regulatory Conference or provide a written response, but decided not to contest the characterization of the risk significance of this finding.

A Notice of Violations is included based on the Licensees failure to meet two Technical Specifications. Specifically, the Licensee: 1) failed to establish adequate procedures for securing FW long-path flushing; and, 2) exceeded the Limiting Conditions for Operation for Primary Containment Isolation Valves and Reactor Core Isolation Cooling System.

It should be noted that the licensee has not been specifically informed of the enforcement action.
