SUMMARY

On September 28, 2011 an out-of-sequence Emergency Worker Monitoring and Decontamination drill was conducted followed by an out-of-sequence Reception Center drill on September 29, 2011 in association with Grand Gulf Nuclear Station (GGNS), located in Port Gibson, Mississippi. Personnel from the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA) Region VI evaluated all activities. The purpose of the drill was to assess the level of preparedness of local responders to react to a simulated radiological emergency at GGNS. The previous plume exercise was conducted on September 9, 2009.

Personnel from the State of Louisiana, Tensas Parish, Concordia Parish, and Grand Gulf Nuclear Station participated in the drills. Cooperation and teamwork of all the participants was evident during the drill and DHS/FEMA wishes to acknowledge these efforts.

This report contains the final evaluation of these out-of-sequence drills. The participants demonstrated knowledge of their emergency response plans and procedures. There were no Deficiencies identified. One Plan Issue was identified and corrected following the demonstration. Two Area Requiring Corrective Action (ARCA) were identified and corrected during the drill.

SUMMARY

2.1 Exercise Purpose and Design The DHS/FEMA Region VI Radiological Emergency Preparedness staff evaluated the drills on September 28 & 29, 2011 to assess the capabilities of local emergency preparedness organizations in implementing their Radiological Emergency Response Plans and procedures to protect the public health and safety during a radiological emergency involving Grand Gulf Nuclear Station (GGNS). The purpose of this report is to present the results and findings on the performance of the offsite response organizations during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities Exercise objectives and identified Capabilities/REP Criteria selected to be exercised are discussed in the Exercise Plan (EXPLAN), Appendix C.

2.3 Scenario Summary Each drill scenario was developed to evaluate the response of drill participants to an incident requiring evacuation of the public from the Louisiana portion of the 10-mile Emergency Planning Zone surrounding the Grand Gulf Nuclear Station. The drill scenario provided for the evaluation of the Concordia Parish Reception Center and Tensas Parish Emergency Worker Monitoring and Decontamination was designed to test the ability of participants to conduct monitoring, decontamination and registration of evacuees and emergency workers and their equipment. A separate walk-through of the congregate care facility was conducted to ensure that services and accommodations provided at the Ferriday High School were consistent with established American Red Cross guidelines.

After Action Report/Improvement Plan Grand Gulf Nuclear Station SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Drill Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the September 28 & 29, 2011 drill evaluations to test the offsite emergency response capabilities of local governments in the 10-mile Emergency Planning Zone surrounding the Grand Gulf Nuclear Station.

Each jurisdiction and functional entity was evaluated on the basis of its demonstration of criteria delineated in the exercise evaluation areas as outlined in the April 25, 2002, Federal Register, Radiological Emergency Preparedness: Evaluation Methodology. Detailed information on the exercise evaluation area criteria and the extent of play agreement used in this drill are found in Appendix D of this report.

3.2 Summary Results of Drill Evaluation The matrix presented in Table 3.1 on the following page, presents the status of all exercise evaluation area criteria that were scheduled for demonstration during these drills by all participating jurisdictions and functional entities. Exercise criteria are listed by number and the demonstration status is indicated by the use of the following letters:

POSSIBLE CAUSE: The Monitor made a mathematical mistake when calculating the reading on the x10 scale.

CONDITION: Members of the Personnel Decontamination Team were unable to determine correct background readings and contamination survey readings on their Ludlum survey meter.

POSSIBLE CAUSE: Team members performed the mathematical calculations on differing scales incorrectly.

REFERENCE:

NUREG 0654 J.12; K.5.a, FEMA REP-22 EFFECT: Incorrect readings that were lower than the actual readings were used.

This miscalculation could cause contamination to go undetected leading to the spread of contamination and increased exposures to the evacuee and others.

CORRECTIVE ACTION DEMONSTRATED: In each case where the Monitor or another member of the team mis-read the survey meter, the controller stopped play and provided training to the team member. The team member then demonstrated the proper way to read the survey meter.

ISSUE NO.: 28-11-6a1-P-03 CRITERION: Reception center/emergency worker facility has appropriate space, adequate resources, and trained personnel to provide monitoring, decontamination, and registration of evacuees and/or emergency workers. (NUREG-0654, J.10.h.,

K.5.b)

CONDITION: The Personnel Contamination Survey Report form, showing the evacuee is free of contamination, was not given to the evacuee prior to releasing him from the Personnel Decontamination Station to the registration area of the reception center. A member of the team realized that the form was not provided and quickly took it to the evacuee.

POSSIBLE CAUSE: Current procedures do not include directions to provide the Personnel Contamination Survey Report form to the evacuee before releasing him to register.

REFERENCE:

NUREG 0654 J.12 EFFECT: An evacuee will not be allowed to register or enter a shelter without either a clean hand stamp or a Personnel Contamination Survey Report form showing the individual is free of contamination.

CORRECTIVE ACTION DEMONSTRATED: Revised procedures were provided to include instructions that the Personnel Contamination Survey Report form be provided to the evacuee prior to releasing him from the Personnel Decontamination Station. In addition, the evacuee will be provided specific instructions on where to go once he enters the reception center registration area.

After Action Report/Improvement Plan Grand Gulf Nuclear Station SECTION 4: CONCLUSION Based on the results of the drills, the offsite radiological emergency response plans and preparedness for the State of Louisiana and the affected local jurisdictions are deemed adequate to provide reasonable assurance that appropriate measures can be taken to protect the health and safety of the public in the event of a radiological emergency. Therefore, 44 CFR Part 350 approval of the offsite radiological emergency response plans and preparedness for the State of Louisiana site-specific to GGNS will remain in effect.

After Action Report/Improvement Plan Grand Gulf Nuclear Station I. PURPOSE This simulated radiological event is being conducted in order to exercise both the emergency worker monitoring and decontamination in Tensas Parish and the public reception center monitoring and decontamination in Concordia Parish for Grand Gulf Nuclear Station (GGNS). The basic objective is to assess the ability of the emergency workers to monitor and decontaminate the public and other emergency workers.

II. PROPOSED SCHEDULE A. Date: September 28, 2011 Location: Concordia Parish Reception Center Ferriday High School Ferriday, LA Time: 6:00 pm B. Date: September 29, 2011 Location: Tensas Parish Emergency Worker Monitoring & Decontamination Tensas Parish Emergency Operations Center 205 Hancock Street St. Joseph, LA Time: 6:00 pm III. SCENARIO A General Emergency has been declared at Grand Gulf Nuclear Station in Port Gibson, MS.

IV. DISCLAIMER Real life emergencies take precedence over this demonstration. In case of an emergency, the demonstration will be cancelled and rescheduled for a later date.

Evaluation Area 1: Emergency Operations Management Sub-element 1.e - Equipment and Supplies to Support Operations Criterion 1.e.1: Equipment, maps, displays, dosimetry, potassium iodide (KI), and other supplies are sufficient to support emergency operations.

i. Personnel will be pre-staged at or near the facility.

ii. The capability to monitor 20% of the Tensas Parish evacuees living in the 10-mile EPZ over a 12-hour period will be demonstrated. To meet 20% requires (20% of 3,625 population) a monitoring capability of 1 evacuee per minute.

iii. Six people will simulate evacuees for demonstration. One portal monitor will be set up and the 6 evacuees processed.

One of the six will be simulated contaminated. One monitoring station for contaminated evacuees will be set up and the contaminated individual will be processed to the decontamination station. Decontamination will be evaluated by demonstration and further discussion.

iv. Handling of potentially contaminated personal belongings will be demonstrated by discussion.

v. Note: The reception center emergency workers will setup the male monitoring/decontamination plan for ease of access during the drill. However, the gender monitored and decontaminated will likely be mixed during this demonstration.

vi. One vehicle will be monitored for contamination. The vehicle will not proceed to the segregated parking area after monitoring. The segregated parking area will not be set up to receive vehicles, but will be available for viewing and discussion.

vii. A roster indicating 24-hour coverage for the reception center will be available. A shift change will not be demonstrated at this location, but may be discussed out of sequence with the Emergency Manager/Officer-in-Charge.

Follow along with the contamination diagram (see Cue Card #6B) while the emergency workers monitor the individual and find contamination. When they reach locations of contamination, inject the cue cards with the Ludlum 3 survey meter face readings.

NOTE: Caution must be taken to give the cue card with the matching scale. If the survey meter is on X10, then the X10 cue card must be given even if they should have been X0.1. Multiple cue cards are available dependent upon the scale setting.

Anticipated Response: The Initial Personnel Monitor should give details of locations and readings to the Initial Personnel Monitor Recorder.

The Initial Personnel Monitor Recorder will ask the contaminated emergency worker questions and complete the Personal Contamination Survey, put the form in a plastic baggie, and direct the individual to the Personnel Decontamination station.
