NRC Office of Investigations Case No. 1-2010-042 The Region I Field Office, NRC Office of lnvestigations (Ol), initiated an investigation (Case No.
1-2010:042) on June 22,2010, to determine whether personnel in the Radiation Protection and Mechanical Maintenance Departments had signed-off on maintenance activities that they did not personally perform or witness being performed. Based on the evidence developed by Ol, tne
'runC was unable to substantiate thai personnel in the Radiation Protection and Mechanical Maintenance Departments had signed-off on maintenance activities that they had not personally performed or witnessed.
As part of its investigation, Ol selected a smart sample of employees to interview in an attempt to ascertain whethei there was any such misconduct. Under sworn testimony, each employee testified that during their careers, they only signed-off on work that they had performed, observed, and/or supervised, and that this included work that involved the Radiation Protection Department. As a result, based on the totality of the testimony obtained during the investigation, including the absence of any specific instance of an improper signature being providid or obtained for work packages and/or work orders, ol found insufficient evidence to conclude that there was falsification of any such records' please note that final NRC investigation documents, such as the Ol report described above, may be made available to the public under the Freedom of Information Act (FOIA) subject to redaction of information appropriate under the FolA. Requests under the FolA should be made in accordance with 10 CFR 9.23, Requests for Records, a copy of which is enclosed for your information.
