(1) Initial startup and approach to power; (2) All fuel movement or relocation; (3) All control rod relocations within the core region (4) Relocation of any in-core experiments or irradiation facilities with a reactivity worth greater than $1.00; (5) Recovery from unplanned or unscheduled shutdown; or (6) Recovery from unplanned or unscheduled significant power reduction.
