VIRGINIA ELECTRIC AND POWER COMPANY MILLSTONE POWER STATION UNITS 1, 2 AND 3 SURRY POWER STATION UNITS 1 AND 2 NORTH ANNA POWER STATION UNITS 1 AND 2 FITNESS-FOR-DUTY PROGRAM; ANNUAL PERFORMANCE DATA REPORT REVISION OF FATIGUE MANAGEMENT INFORMATION Pursuant to 10 CFR 26.717 (e), on February 28, 2011, Dominion Nuclear Connecticut, Inc. (DNC) and Virginia Electric and Power Company (Dominion) submitted the Annual Fitness-For-Duty (FFD) Program Performance Data report for January 1, 2010 through December 31, 2010, as part of a Dominion fleet report. In a telephone discussion on April 28, 2011, the NRC indicated that the submitted fatigue management information for Millstone and Surry had not accounted for individual waivers as was intended by the NRC. Further, the NRC requested verification whether the applicable North Anna waiver worksheets had been intentionally submitted blank.
During the subsequent re-verification of the performance worksheets, several additional entries were identified as missing from the submitted Millstone and Surry information.
Enclosed are replacement pages for the Fatigue Management worksheets submitted for Surry and Millstone Power Stations. These replacement pages include the missing and revised data entries discussed above. In addition, the waiver worksheets submitted for North Anna are hereby verified as having been submitted intentionally blank.

There were no corrective actions associated with the waivers at Millstone. All fatigue assessments associated with waivers found the individuals were not impaired from fatigue.
