21.21 UNSPECIFIED PARAGRAPH PRESSURE SWITCH WITH POSSIBLE SUB-STANDARD ELECTRICAL LEAD TERMINATIONS SOR Process Instruments has determined that certain model pressure switches it has manufactured may have leads attached to the terminal blocks that are not in conformance with good manufacturing practice. The pressure switches in question may have lead wire attached to the terminal block in which the connection between the stripped, stranded wire and a crimped terminal may not be flush with the terminal end. SOR states that there have been no known field problems associated with this issue.
This condition applies to SOR model switches with terminal block "X" options 9013-737, 9013-747, 9013-7674.
SOR has provided this information to those customers that purchased these switches. The U.S. plants that received these switches are Braidwood and Duane Arnold.

This is an information notice regarding SOR pressure switches that have the electrical terminal block option. A condition of general good manufacturing practice has been brought to our attention in regards to the crimping of wire lugs onto lead wires in the SOR terminal block assembly. SOR allowed a switch to be shipped in which one of the three lead wires in this switch did not meet these criteria, see attached picture.
scope:
This information notice applies to SOR models with the terminal block "X" options, 9013-737, 9013-747 and 9013-764. An attachment identifies each switch by your P0 number, the SOR sales order number, SOR model number, SOR serial number and the ship date. This notice does not apply to SOR switches that have the SCR factory conduit seal "JJ" option.
Discussion:
Good manufacturing practice for creating a connection between a stripped, stranded wire and a crimped terminal suggests that the stranded wire be visible inside the lug after crimping. The wire may be flush with the end of the terminal or protruding out no more than 1/16". In one instance, one of the three wires on a terminal block did not have bare wires that were flush with the terminal end. We believe this was a random occurrence, however, we decided to bring this to your attention. In addition, all switches at SOR must pass a number of tests before they are released for shipment. All of these tests are documented by Production personnel and witnessed by Quality Control personnel. All pressure switches that are in your possession now have passed these tests. There have been no known field problems associated with this issue.
Customer Action:
At our customer's option, the subject products may be visually inspected and any wire connections that do not meet the noted criteria may be returned to SOR for rework. To return a switch, contact SOR for a Returned Material Authorization (RMA) number. SOR will pay freight and replace the terminal free of charge. If you decide to inspect the switches, SOR will send replacement cover gaskets. Please notify us if you wish to have gaskets shipped to you.
Preventative Action:
We have instituted improved procedures and training for Fabrication, Assembly and Inspection personnel to prevent future incidences.
