OF THE ISSUES There are 50 SWEC eoncerns in this subcategory. The 50 concerns were grouped into 27 issues for the effective evaluation of similar concerns. The verification activities for the 50 concerns were documented in 27 BLN SWEC Element files. These files established the background, corrective action, and completion status for each SWEC issue.
The overall sunmary process of evaluation, and significance of the SWEC issues is discussed in this SWEC subcategory report.
-II. MAJOR FINDINGS Summary data of the SQN SWEC.issues is as follows:
21 issues are statused as closed.
- 8 issues are statused as having completed corrective action.
These issues will be considered closed following NRC closure.
- 21 issues are statused as having satisfactory corrective action in progress.
- No issues are statused as having insufficient evidence of corrective action.
III.
COLLECTIVE SIGNIFICANCE OF MAJOR FINDINGS All of the SWEC issues were factually accurate and required corrective action by BLN.
This corrective action was verified by ECTG to be complete or satisfactorily in progress for all BLN SWEC issues.
IV.
CAUSES OF THE MAJOR FINDINGS The four leading causes of the SWEC concerns were (1) procedure compliance inadequacy (2) procedural inadequacy (3) supervision inadequacy, and (4) lack of corrective action (including timeliness).
V. CORRECTIVE ACTION ON MAJOR FINDINGS ECTG verified that corrective actions had been completed or were in progress for all of the 50 SWEC issues at BLN.

2 PAGE i OF viii Preface This subcategory report is one of a series of reports prepared for the Employee Concerns Special Program (ECSP) of the Tennessee Valley Authority (TVA). The ECSP and the organization which carried out the program, the Employee Concerns Task Group (ECTG), were established by TVA's Manager of Nuclear Power to evaluate and report on those Office of Nuclear Power (ONP) employee concerns filed before February 1, 1986.
Concerns filed after that date are handled by the ongoing ONP Employee Concerns Program (ECP).
The ECSP addressed over 5800 employee concerns. Each of the concerns was a formal, written description of a circumstance or circumstances that an employee thought was unsafe, unjust, inefficient, or inappropriate. The mission of the Employee Concerns Special Program was to thoroughly investigate all issues presented in the concerns and to report the results of those investigations in a form accessible to ONP employees, the NRC, and the general public. The results of these investigations are communicated by four levels of ECSP reports: element, subcategory, category, and final.
Element reports, the lowest reporting level, will be published only for those concerns directly affecting the.restart of Sequoyah Nuclear Plant's reactor unit 2. An element consists of one or more closely related issues. An issue is a potential problem identified by ECTG during the evaluation process as having been raised in one or more concerns. For efficient handling, what appeared to be similar concerns were grouped into elements early in the program, but issue definitions emerged from the evaluation process itself. Consequently, some elements did include only one issue, but often the ECTG evaluation found more than one issue per element.
Subcategory reports summarize the evaluation of a number of elements.
However, the subcategory report does more than collect element level evaluations. The subcategory level overview of element findings leads to an integration of information that cannot take place at the element level. This integration of information reveals the extent to which problems overlap more than one element and will therefore require corrective action for underlying causes not fully apparent at the element level.
To make the subcategory reports easier to understand, three items have been placed at the front of each report: a preface, a glossary of the terminology unique to ECSP reports, and a list of acronyms (terms formed from the first letters of a series of words).
Additionally, at the end of each subcategory report the reader will find at least two attachments.
The first is a Subcategory Summary Table that includes the following information:
the concern number, a brief statement of the concern, and a designation of nuclear safety-related concerns. The second attachment is a listing of the concerns included in each issue evaluated in the subcategory.

SUMMARY
The nurose of ECTG's verification activities for the SWEC issues was to 1.0 CHARACTERIZATION OF ISSUES 1.1 Introduction The Stone & Webster Engineering Corporation (SWEC) Systematic Analysis of Identified Issues/Concerns at TVA was performed by a group of 22 senior-level Stone & Webster personnel from January 20 January 31, 1986 at TVA. Background information on the SWEC analysis was provided by SWEC in their report transmittal of February 14, 1986 (J. A. Kirkebo to L. D. Nace):
In a meeting with the NRC Commissioners on January 9, 1986, TVA Directors committed to a review and evaluation of previously identified issues/concerns as a means of assessing the current situation as well as identifying certain root causes of problems. To meet this commitment a Systematic Analysis was to accumulate issues and concerns from sources external to TVA, encode these into a data base, analyze the resultant information, determine root causes where possible and support the preparation of a report to NRC outlining the TVA Recovery Plan.
The sorts of the resulting data base were utilized as a foundation for preparation of the revised volume 1 of the TVA Nuclear Performance Plan.
All issues/concerns accumulated by SWEC were turned over to the Employee Concerns Task Group (ECTG) for evaluation.
1.2 Description of Sources Stone & Webster identified issues and concerns from TVA's incoming correspondence over a 16-month period ending January 1986. ECTG determined that the sources of the issues and concerns for Bellefonte Nuclear Plant (BLN) were as follows:
38 issues identified by the Nuclear Regulatory Commission (NRC) including violations, inspector follow-up items, unresolved items, and unnumbered concerns.
- Seven issues included inNRC's 1985 Systematic Assessment of Licensee Performance (SALP) Reports
- Three issues identified by the Institute of Nuclear Power Operations (INPO)
- Two issues identified in correspondence from the Tennessee Congressional delegation.
1.3 Description of Issues Because of the variety and types of issues covered, it is inappropriate to discuss individual issues in the body of this report. Details of each issue are included in Attachment A of this report.
8 other issues were statused as having completed corrective actions. These issues will be considered closed following closure by the NRC. Corrective Action Tracking Documents (CATDs) for these issues will not be issued by ECTG.
- 21 issues were statused as having satisfactory corrective action in progress. CATDs for these issues are listed in section 4.2.
- No issues were statused as having insufficient evidence of corrective action.

7.0 CORRECTIVE ACTION As noted above, all of the SWEC issues were factually accurate and required corrective action by TVA. Corrective action for 29 of the 50 SWEC issues had been completed at the time of the ECTG verification activity. An additional 21 issues verified as having corrective action in progress are listed in section 4.2.
The corrective action status of all issues is identified in Attachment A of this report.
