Management Actions Summary A summary of management actions reveals a total of 50 program violations during the year 2010. This includes six (6) involving Dominion employees (3 Random for alcohol, 2 Random for illegal drugs, and 1 For Cause for alcohol). Forty-four (44) involved contract supplemental employees {34 Pre-Access (22 for illegal drugs, 6 for alcohol, and 6 for test refusal); 1 For Cause for alcohol; 2 Post-Event (1 for alcohol and 1 for test refusal); 6 Random (4 for illegal drug, 1 for alcohol, and 1 for test refusal);

Summary of Events and Management Actions Completed to Improve the FFD Program January 1 Through December 31,2010 Effective April, 2010, Dominion fleet (Kewaunee, North Anna, Millstone and Surry Power Station) FFD records were centralized at the Innsbrook corporate facility. Records of positive test results that were previously maintained at the sites within the fleet were relocated to the corporate office. This change provides improved oversight of these records prior to final archiving. In addition, the Medical Review Officer, (MRO) now maintains an office at the corporate FFD office. This has facilitated better internal communications concerning FFD issues.

In conjunction with the aforementioned change, the FFD Program Supervisor and Lab Testing Supervisor now function as members of the MRO staff. This organizational reporting change facilitates for faster and more efficient processing of FFD results upon retrieval from the HHS web site.

On March 31,2010, the Substance Abuse Expert (SAE) portion of 10 CFR 26 was implemented. Dominion has revised its procedures and contracted with its Employee Assistance Program (EAP) to provide substance abuse evaluations. In addition, the Medical Review Officer was trained as a substance abuse expert (SAE) and serves as the final reviewer for substance abuse evaluations.

There were no corrective actions associated with the waivers at Millstone. All fatigue assessments associated with those waivers found the individuals were not impaired from fatigue.

List the affected group, reason for the assessment, and the action taken On 10/3/10, a Security Officer(SO) Self-Declared during an Outage. A Fatigue Assessment was performed and determined SO was able to drive and employee was released to drive home. CR 40973 was subsequently inititaed to identify this was not in compliance with L1-AA-700. Only part of page on of the Fatigue Assessment form is required to be completed for a Self-Declaration, and this is only for record purposes since the procedure does not require a Fatigue Assessment to be performed. In addition the procedure requires that the individual be driven home instead of being allowed to drive themselves home.

On 11/9/10, during a non-outage period CR403077 was initiated to report an optional Fatigue Assessement (FA) was performed for an Operator who had exceeded the 72/168 rule, to verify he was okay to drive home and no fatigue issues were identified. This FA was not required by L1-AA-700 and is documented here for accountability purposes only.
