Enclosed is one copy of the After Action Report / Improvement Plan for the October 26, 2010, Radiological Emergency Preparedness (REP) Full Participation Plume Exposure Pathway Exercise for the Point Beach Nuclear Plant. Under separate cover, four copies of this report are being sent to the Wisconsin Emergency Management Administrator for distribution to the Counties of Kewaunee and Manitowoc along with an additional copy for the state. The State of Wisconsin, Kewaunee and Manitowoc Counties, and the utility owner/operator, NextEra Energy, participated in this exercise.
The After Action Report / Improvement Plan was prepared by the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA) Region V, Radiological Emergency Preparedness Program.
No Deficiencies were identified for any jurisdiction during this exercise.
There were four Areas Requiring Corrective Action (ARCAs) identified for the State of Wisconsin during this exercise; one of these ARCAs was successfully redemonstrated during the exercise.
The first ARCA for the State of Wisconsin was identified under Criterion 1. e. 1 - Equipment, maps, displays, dosimetry, potassium iodide, and other supplies are sufficient to support emergency operations - whereby the Mobile Radiological Laboratory did not conduct pre-operational checks of their survey instruments and used instruments that did not have calibration or operating range labels.
The second ARCA for the State of Wisconsin was identified under Criterion 5. b. I - Emergency Information and Instructions to the Public and the Media - whereby the State Emergency Operations Center (SEOC) issued a press release instructing the public to call the Joint Information Center (JIC)
Public Inquiry Hotline approximately one half hour before the hotline staff was operational.
The third ARCA for the State of Wisconsin was identified under Criterion 6. a. I - Monitoring and Decontamination of Evacuees and Emergency Workers - whereby staff at the Manitowoc County Highway Department Reception Center received inadequate training and/or practice on procedures and demonstrated improper use of survey instrumentation. This ARCA was successfully redemonstrated www.fema.gov
NRC Headquarters Document Control Desk Page 2 during the exercise. The State is required to submit corrected procedures regarding the proper use of instrumentation for review within ninety (90) calendar days of the October 26, 2010 Exercise.
The fourth ARCA for the State of Wisconsin was identified under Criterion 1. c. I - Direction and Control - whereby State Field Team briefings were not conducted by the Field Team Leader due to inadequate communication and coordination between the Field Response Manager and Field Team Leader in the Forward Operating Center.
There were two prior ARCAs and a Planning Issue identified for the State of Wisconsin that were resolved during this exercise. These issues included Criterion 1.c. I - Direction and Control, 6. a. 1 -
Monitoring.nd Decontamination of Evacuees and Emergency Workers, and Criterion 2. b. 2 - A decision making process involving consideration of appropriate factors and necessary coordination is used to make PADs for the general public (including KI, if ORO policy).
The first ARCA from a previous exercise which was resolved by the State of Wisconsin was identified under Criterion 1. c. 1 - Direction and Control - whereby a lack of adequate direction and control resulted in team performance being less than optimal because there were no team briefings and no opportunities were taken to have a group discussion of key issues at hand.
The second ARCA from a previous exercise which was resolved by the State of Wisconsin was identified under Criterion 6. a. 1 - Monitoring and Decontamination of Evacuees and Emergency Workers - whereby staff at the Manitowoc County Highway Department Vehicle Monitoring and Decontamination Station received inadequate training and/or practice on procedures and demonstrated improper use of survey instrumentation.
The Planning Issue from a previous exercise which was resolved by the State of Wisconsin was identified under Criterion 2. b. 2 - A decision making process involving consideration of appropriate factors and necessary coordination is used to make PADs for the general public (including KI, if ORO policy) - whereby the State Emergency Operations Center staff used checklists and guidance during the General Emergency that did not accurately reflect the process or steps by which a Protective Action Recommendation (PAR) becomes a Protective Action Decision (PAD) in a "home rule" state.
There were no new issues identified for Kewaunee and Manitowoc Counties during this exercise.
Based on the results of the October 26, 2010, exercise, the offsite radiological emergency response plans and preparedness for the State of Wisconsin and affected local jurisdictions, site-specific to the Point Beach Nuclear Plant, can be implemented and are adequate to provide reasonable assurance that appropriate measures can be taken offsite to protect the health and safety of the public in the event of a radiological emergency at the site.
Therefore, the Title 44 CFR, Part 350, approval of the offsite radiological emergency response plans and preparedness for the State of Wisconsin site-specific to the Point Beach Nuclear Plant, granted on June 14, 1985, remains in effect.

SUMMARY
On October 26, 2010, a Radiological Emergency Preparedness (REP) Plume Exposure Pathway Exercise was conducted for the 10-mile Emergency Planning Zone (EPZ) around the Point Beach Nuclear Plant (PBNP) by the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA), Region V. The purpose of the exercise was to assess the level of State and local preparedness for response to a radiological emergency. This exercise was conducted in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures.
A Radiological Emergency Preparedness (REP) Full Participation Plume and Ingestion Exposure Pathway Exercise was scheduled for December 9 and 10, 2008 in the 10-mile Emergency Planning Zone (EPZ) around the Point Beach Nuclear Plant (PBNP). The exercise was terminated early by the State of Wisconsin due to a real-world weather emergency. The termination occurred prior to PBNP notification to the State that the site had declared a General Emergency (GE) Emergency Classification Level (ECL). The early termination precluded the completion of virtually all activities for both the plume and ingestion phases of the exercise that were scheduled for December 9 and 10, 2008.
A determination was made by DHS/FEMA Headquarters that credit would only be given for the out-of-sequence activities completed on December 8, 2008, in effect reducing demonstrated activities to a REP Partial Participation Plume Exposure Pathway Exercise. The exercise activities for which credit was given were held in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local emergency operations plans and procedures.
The previous complete Plume Exposure Pathway exercise was conducted on December 5, 2006.
The qualifying emergency preparedness exercise for the PBNP was conducted on March 9, 1982.
DHS/FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise. The State of Wisconsin, the risk counties of Kewaunee and Manitowoc, local municipalities as well as various non-government entities and volunteers all contributed to the success of the exercise.
Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.

SUMMARY
2.1 Exercise Purpose and Design The Point Beach Nuclear Plant (PBNP) Biennial Radiological Emergency Preparedness (REP)
Full Scale Participation Plume Exposure Pathway exercise was sponsored by NextEra Energy Resources in conjunction with the State of Wisconsin and the Counties of Manitowoc and Kewaunee. DHS/FEMA Region V evaluated the PBNP REP Full Participation Plume Exposure Pathway exercise conducted on October 26, 2010, to assess the capabilities of local emergency preparedness organizations to implement their Radiological Emergency Response Plans (RERP) and procedures in order to protect the public health and safety during a radiological emergency involving the PBNP. The purpose of this report is to present the results and findings regarding the performance of the Offsite Response Organizations (OROs) during a simulated radiological emergency.
The PBNP REP Plume Exposure Pathway exercise was an unclassified exercise. The control of information is based more on public sensitivity regarding the nature of the exercise than on the actual exercise content. In addition to demonstrating the exercise evaluation criteria prescribed in Federal Regiser Notice Vol. 67, No. 80, dated April 25, 2002, the PBNP REP Full Plume Exposure Pathway exercise also provided a forum to test and familiarize participants at all levels with the materials, processes and procedures associated with the DHS/FEMA Homeland Security Exercise and Evaluation Program (HSEEP).
The Exercise Planning Team (EPT) was comprised of representatives from the participating Federal, State and local jurisdictions who conducted both the normal REP exercise planning and preparations and developed HSEEP specific Exercise Plan (ExPlan), Controller and Evaluator Handbooks and Exercise Evaluation Guides (EEGs). The dual objectives of satisfying statutory evaluation requirements and introducing the HSEEP evaluation process would not have been possible without the extensive collaboration and cooperation of all parties involved.

Appendix F "Exercise Plan" contains a summary of the Exercise Scenario, a sequence of simulated events that was used as the basis of initiating emergency response actions by the Offsite Response Organizations (OROs) during the Point Beach Nuclear Plant (PBNP) REP Full Participation Plume Exposure Pathway exercise on October 26, 2010.
This exercise scenario was submitted by the State of Wisconsin and NextEra Energy on August 23, 2010. The orignal field data was modified to reflect plume travel and the revised scenario was resubmitted on Septeember 27, 2010. The revised scenario was approved by DHS/FEMA i
Region V on October 4, 2010.
During the exercise, in addition to information and data provided through the Point Beach I
Nuclear Plant (PBNP) on-site scenario, controllers from the State of Wisconsin provided inject messages containing scenario events and/or relevant data to the individuals and/or locations that i
would normally recieve notification of such events. These inject messages were the method used to initiate specific action for the OROs.

After Action Report/Improvement Plan Point Beach Nuclear Plant SECTION 3: ANALYSIS OF CAPABILITIES 3.1 Exercise Evaluation and Results This section contains the results and findings of the evaluation of all jurisdictions and functional entities that participated in the October 26, 2010, REP Plume Exposure Pathway Exercise to test the off-site emergency response capabilities of State and local governments in the 10-mile EPZ surrounding the PBNP.
Each jurisdiction and functional entity was evaluated based on its demonstration of exercise criteria delineated in Federal Register Notice/Vol. 67, No. 80, dated April 25, 2002. Detailed information on the exercise criteria and the extent of play agreement used in this exercise are found in Appendix 3 of this report.

Callers were provided telephone numbers for, and instructed to call, either the Point Beach Manager of Communications or the Supervisor of the Point Beach Energy Center. This can be confirmed by calling the Point Beach Hotline.

ISSUE NO.: 50-10-2b2-P-01 ISSUE: During the Prairie Island Nuclear Generating Plant (PINGP) Radiological Emergency Preparedness Exercise on Tuesday, August 24, 2010, the Officer-in-Charge (OIC) and the Operations Manager and staff for the State of Wisconsin used a checklist to guide their actions during a General Emergency which did not reflect the processes or steps required for a Protective Action Recommendation (PAR) to become a Protective Action Decision (PAD) in a "home rule" state.
The State and its resources support County decision making by providing a PAR (for evacuations, sheltering in place) based on information received from PINGP, the efforts of field monitoring teams, and associated projections (see Criterion 2.b.1).
The County then makes a decision (PAD) based on the PAR provided by the State.
In the checklist entitled, "OIC/Operations Actions, General Emergency", after concurrence is reached on the substance of protective action and "counties agree on a time to sound sirens", the OIC is to "request approval of the PAR from the Governor or Governor's Representative, and the PAR becomes a PAD."
Contrary to the (sequence of) steps in the checklist, the PAR does not become a PAD upon the Governor Representative's approval. The PAR becomes a PAD upon the acceptance of the PAR by Pierce County, as perhaps modified by them to account for local considerations. The Governor or Governor's Representative certainly can approve a PAR, but in a "home rule" state, they are not in the role of approving the County's actions in this regard. As a "home rule" state, Pierce County is responsible for the overall decision to make a protective action (e.g., evacuation, shelter-in-place). Also, the language in the checklist did not entirely reflect the coordination 25

The State of Wisconsin Forward Operating Center (FOC) demonstrated the Target Capability of.
On-Site Incident Management by coordinating radiological sampling operations required to support Protective Action Decision-making during the PBNP REP Exercise. The FOC was well equipped with all necessary communications, equipment and supplies required for the Field Response Manager (FRM) to maintain situational awareness and direct Restricted Area Field Team (RAFT) operations. Based on direction from the SRC and current meteorological conditions, the FRM and Field Team Coordinator (FTC) effectively deployed two RAFTs and communicated specific sampling orders to monitor the size and magnitude of the simulated radiological release. Field sampling data were relayed to the Mobile Radiological Laboratory (MRL) and SRC in a timely manner. An issue was identified in which the FRM became directly involved in RAFT operations and FOC briefings were not performed in accordance with the FOC/MRL operations manual. After Action Report/Improvement Plan Point Beach Nuclear Plant POSSIBLE CAUSE: Possible Cause: Inadequate procedures, training and/or practice with the instrument and its use.
The Auxiliary Health Monitors Standard Operating Procedure, dated August, 2010 used for this exercise was formatted in a way that requires survey staff to flip between multiple pages to assemble the instructions needed to perform adequate monitoring. The requirement to keep the instrument probe one half inch from surface and moved at approximately one inch per second and that the survey of a person should take approximately three minutes is on page ATT 1-17. That the probe covering should fit tight and not hang down and that background readings should be taken every half hour is on page ATT 1-19, and Appendix A on page ATT 1-27 that is labeled "Survey Procedure - Ludlum Model 3" contains none of this information.
The only reference to cross contamination occurs on page ATT 1-10 and it in reference to vehicle monitoring. Although the SOP contained the relevant information, it was not organized as an effective Job Aid / ready reference for Auxiliary Health Monitors performing radiological surveys during an emergency.

EFFECT: During the monitoring and decontamination of the first contaminated evacuee the Personnel Decontamination Staff processed, the operator of the Ludlum 3 survey meter with pancake probe allowed the protective plastic bag covering the probe to touch/drag across the clothing of the evacuee on four separate instances.
This could have spread contamination on the evacuee and caused incorrect readings with the meter. The operator did not self-identify this mistake, nor did the Health Team Leader assisting her. The operator then set the Ludlum meter on the floor in the designated dirty area of the locker/shower room and surveyed the jacket of the evacuee while holding directly over the meter. Any loose contamination on the jacket could have been shaken directly onto the instrument again spreading contamination.
RECOMMENDATION: The controller and staff conducted remedial training and the same operator performed monitoring duties on two separate evacuees and one emergency worker identified as possibly contaminated with no inadequacies noted.

ABMC demonstrated redundant communications systems'and effective patient management between the Emergency Department (ED) and ambulances. RECOMMENDATION: The State EOC PIO should verify with the JIC that the Public Information Hotline is staffed and operational before issuing a news release directing the public to contact the Hotline regarding questions and concerns. Additionally, the State EOC PIO should ensure that County EOC PlOs are provided appropriate handling instructions for any public informantion documents marked "For Immediate Release." Public Information Officer plans, procedures, guides and checklists should be reviewed and updated, as appropriate, to incorporate the verification process.

RECOMMENDATION: Install calibration labels indicating acceptable ranges of operation during the calibration process and train personnel on the proper procedure for performing operability checks in accordance with procedures.
CORRECTIVE ACTION DESCRIPTION: The following actions will be taken in response to ARCA #49-l0-1el-A-01 issued against the State of Wisconsin, Mobile Radiological Laboratory. Acceptable ranges of operation were not posted on the calibration stickers on the Bicron Micro-Rem meters or the Ludlum Model 12 count rate meters used at the Mobile Radiological Laboratory or the Sample Receiving Area nor was the Field Response Manager contacted for assistance because there was no means to ensure the instrument was operating properly.
Although the State of Wisconsin does not have an immediate solution for the Bicron Micro-Rem meter issue, the Wisconsin Department of Health Services - Radiation Protection Section is in the process of conducting research and discussing approaches taken by other states for a solution that will satisfy all stakeholders. It is anticipated that this research can be conducted within the next 30 days. The State of Wisconsin will send FEMA a subsequent letter describing the solution. Regarding the Ludlum meters, these meters will be sent back to the manufacture for calibration and an appropriate calibration sticker.

This one day exercise is a full participation plume exposure pathway exercise, with participation by the State of Wisconsin Emergency Operations Center (EOC) in Madison, Manitowoc County EOC in Manitowoc, and the Kewaunee EOC in Algoma. The Joint Information Center (JIC) and the Point Beach Call Center are located in Green Bay. The I
reception centers and congregate care centers are located in Kewaunee and Manitowoc counties. In addition, "Aurora Bay Care" hospital in Green Bay, will participate with field operations in Kewaunee County.

Vehicle monitoring and decontamination staff received inadequate training and/or practice on procedures and how to use instruments.
Plans and procedures were inadequate for contamination control, monitoring and decontamination of equipment.
Procedures were missing for re-uniting the evacuees and Emergency Workers with their vehicles, and segregating Emergency Worker vehicles from evacuee vehicles for prioritization. Personnel did not read their DRD's. Finally, there were an inadequate number of personnel available to staff the vehicle monitoring and decontamination stations to complete necessary tasks.

EVALUATION AREA 1 - EMERGENCY OPERATIONS MANAGEMENT N
Criterion 1.a.]: Mobilization: OROs use effective procedures to alert, notify and mobilize 3
emergency personnel and activate facilities in a timely manner.
State of Wisconsin This is a one-day full-participation plume pathway exercise. Decision making and radiological control/dose assessment activities will occur at the State Emergency I
Operations Center (SEOC) and the State Radiological Coordinator (SRC) room (room 105) in Madison, WI. Wisconsin State field sampling teams, will collect samples within the 10 mile Emergency Planning Zone (EPZ) in Manitowoc and Kewaunee Counties.
The initial notification will occur via the Dial Select system to both the State Patrol 3
Headquarters (Warning Center 1) and Wisconsin Emergency Management (Warning Center 2). Dial Select is a dedicated phone system which rings simultaneously at State Patrol Headquarters, WEM Front Desk Receptionist,
- SEOC, the 5
Communications Room located within WEM, and at the Sheriff's dispatch center in both Manitowoc and Kewaunee County. If the initial notification occurs prior to 7:45 a.m., State Patrol will contactthe WEM Duty Officer (DO) via a pager. If the initial notification occurs after 7:45 a.m., the call is taken by WEM front desk staff, who will notify the WEM Duty Officer.
Based upon the WEM standard operating procedure (SOP 4-18), the DO will call the U
SRC. The SRC will then contact Point Beach using the "call-back" number listed on the NARS form, to verify authenticity as well as to get more information.
3 The DO will use the WEM Standard Operating Procedure for activation of the EOC.
As per the Wisconsin Emergency Response Plan, the EOC is activated at the
'ALERT' classification for nuclear power plants that have a 10-mile EPZ within Wisconsin. The SRC will respond in real-time (in sequence) and will report to SRC Room 105, in Madison, once a call back has been made to the plant. EOC security will be demonstrated; exercise participants, observers and evaluators will be asked to wear ID badges.
The DO or his/her designee will employ the use of the EOC software, "E-Sponder" I
for alerting staff internally, as well as external state agencies, and organizations to report to the EOC.
The software sends a pre-scripted message to office phones, cellular phones, and e-mail accounts simultaneously.
Recipients of the notification must respond as to their status (i.e. "press 1 if you plan to report to the EOC," "press 2 if you cannot report to the EOC..."). The DO or his/her designee will monitor the responses by staff.
In the event T1Cat E-Sponder cannot successfully perform its

" Wisconsin Emergency Management Liaisons (Wisconsin Emergency Management Regional Directors) will be pre-positioned at the Kewaunee and Manitowoc County EOCs.
- Public Information Officer staff and Public Inquiry Hotline staff will be pre-positioned in the area of the Joint information Center (JIC). In a real event, the lead PIO may dispatch PIO staff to the JIC at the ALERT.
Twenty-five minutes after the ALERT has been declared, a controller will call the lead PIO and tell them to report to the JIC.
" The Forward Operating Center/Mobile Radiological Laboratory (FOC/MRL) will be pre-positioned at the Two Rivers National Guard Armory.
- Two State restricted area Field Teams (SFTs) will be pre-positioned at a hotel in the city of Manitowoc and will be notified and activated by the State Radiological Coordinator (SRC) to begin play at the appropriate time.
" The MS-1 hospital drill will be conducted at Aurora Bay Care Medical Center out of sequence.
The State will provide a 24-hour staffing list (staff to be identified by positions) to the federal evaluator.
Manitowoc County Activities will occur around Manitowoc County. The base of operations will be the Manitowoc County Emergency Operations Center. Alert and staff call-up procedures will be followed as outlined in the plan using the Joint Dispatch Center (JDC)

The base of operations will be the Kewaunee County Emergency Operations Center, which is located in the lower level of the Algoma City Hall. Alert and staff call-up procedures will b7gollowed as outlined in the plan. The 24-hour shift change staffing list can be seen in the EOC. The Officer-in-Charge (OIC) will determine and announce when the EOC is operational and fully staffed.

response effort for which they are responsible.

The State will demonstrate decision-making capabilities in the SEOC. The State will demonstrate coordination between the SEOC, the JIC, Kewaunee and Manitowoc 3
County EOC's, the utility, and other appropriate off-site response organizations (ORO's) including field operations.
To clear ARCA 33-09-lcl-A-01 the State Radiological Coordinator (SRC) will demonstrate the following. The SRC will conduct briefings and team discussions as a means to gather information, share new developments and provide instructions to staff. The SRC is responsible for conducting an independent assessment of the nuclear plant incident to determine the impact on the public and emergency workers.
In addition, the SRC will assess and evaluate all relevant information as part of the PAR development process.
ARCA 33-09-lcl-A-01 will be cleared by the State Radiological Coordinator (SRC).

After Action Report/Improvement Plan Point Beach Nuclear Plant Final 10/19/2010 Criterion 1.d.J: Communications Equipment: At least two communication systems are available and operate properly and communication links are established with appropriate locations. Communications capabilities are managed in support of emergency operations.
State of Wisconsin The primary communication link between the SEOC and Point Beach Nuclear Power Plant is the Dial Select telephone; for all other locations, the primary means of communication is by commercial telephone.
Traditional phone, fax, and paper systems will be used as primary means of communication, with electronic mail to communicate with others in the State EOC and the JIC, as a secondary means of communication. E-Sponder, WEM's EOC software, will be used for event logging and briefing forms; however, EOC staff are also permitted to handwrite their logs and forms. Backup communications for the State include a variety of other communication systems (e.g., NAWAS, cellular phone, radio, TDD, facsimile, RACES, and satellite telephone), which are used to communicate with Manitowoc and Kewaunee Counties, the utility, and other locations. RACES operators will utilize amateur radio VHF as a backup means of communications between the SEOC and Counties. For the State of Wisconsin, RACES operators will be located in the "Radio Room" which is adjacent to the SEOC.
The Forward Operating Center/Mobile Response Laboratory (FOC/MRL) will communicate between the SRC room and the Field Teams. Communications may be in the form of two-way radio, cellular or VOIP telephone calls and a segregated E-Sponder site (separate from the exercise site). Participants may include the County Radiological Officer Liaison in the SRC room and the Field Team Response Manager at the FOC/MRL.
Manitowoc County Manitowoc County uses a variety of communication systems.
Dial-Select, commercial telephones, cellular telephones, and fax machines will be used to communicate with the utility and other locations. Laptops and e-mail will be used as the primary form of messaging. Paper messaging will be used as a backup or when e-mail is not an option.

Criterion 1.e.):
Equipment and Supplies to Support Operations Equipment, maps, displays, dosimetry, potassium iodide (KI) and other supplies are sufficient to support emergency operations.
State of Wisconsin The State will demonstrate the use of equipment, maps and displays to support emergency operations in the SEOC, Room 105 (the SRC Room) and the JIC. The state will use its EOC software, E-Sponder, and GIS.
The state will demonstrate the availability of dosimetry and potassium iodide (KI).
Documentation of the KI expiration date (February 2014) is stamped on the KI package. Current instrument inventory and calibration records will be verified during a site visit prior to the exercise by the Wisconsin Site Specialist of FEMA Region V.
Manitowoc Count, Manitowoc County will demonstrate their ability to support operations through the use of maps, status boards and other displays as appropriate.
Some county emergency worker dosimetry kits will be pre-distributed and the remainder will be distributed at the Manitowoc County Highway Commission Reception Center during play. The Reception Center will be activated out-of-sequence with the scenario.
Distribution of dosimeter kits with a briefing for the Sheriff's deputies can be observed in the Sheriffs Department Patrol Squad Room and at the traffic control point for the Highway Commission workers. The traffic control point to be evaluated will be identified by the Highway Commission representative in the EOC and the time will be coordinated with the evaluator. KI for emergency workers is stockpiled in the county's EOC. (For this exercise, the first KI dose in the dosimeter kits will be simulated using M&M's). Reception center workers are not instructed to ingest KI.
Dosimeter calibration records and stockpiled KI with expiration date information will be available in EOC.
Instrument inventory and calibration records are maintained by Manitowoc County and will be available for review by an evaluator in the EOC.
Kewaunee Count',
Kewaunee County will demonstrate their ability to support operations through the use of maps, status boards, and other displays as appropriate. The dosimetry kits will be distributed at Luxemburg EMS biading and the County Annex building.
All dosimetry kits contain KI since staff in this group may enter the EPZ as part of their

Staff at both locations will be given a briefing on the use of dosimetry and KI. Documentation regarding the expiration date of the KI will be available for the evaluator in the EOC during the exercise.
Instrument inventory and calibration records are maintained by Kewaunee County and will be available for review by an evaluator in the EOC.
Evaluation Area 2 - Protective Action Decision Making Criterion 2.a.]: Emergency Worker Exposure Control OROs use a decision-making process, considering relevant factors and appropriate coordination, to insure that an exposure control system, including the use of KI. is in place for emergency workers including provisions to authorize radiation exposure in excess of administrative limits or protective action guides.
State of Wisconsin The SRC will be notified of the event in sequence by the WEM Duty Officer and the SRC will report to Wisconsin Emergency Management, Room 105, in Madison. The SRC, based on his/her technical evaluation of the available data, will make recommendations for emergency worker exposure control to the Field Response Manager (FRM) at the FOC/MRL. This includes recommendations for the ingestion of KI that will be made by the SRC to the Kewaunee and Manitowoc County Radiological Officers. Emergency workers, including the field teams, have a dose limit of 3 rem whole body (Deep Dose Equivalent).
The turn-back value for emergency workers is 200mR/hr. For an emergency worker who only has access to a DRD, an exposure rate value of 150mR will be used as the turn back value. By procedure, the SRC can authorize a radiation exposure in excess of the administrative limit for emergency workers.
State field team members will simulate taking KI at the direction of the SRC. KI for field team members is maintained with the field team's field kits.
Kewaunee County Kewaunee County's Radiation Officer (RO).will receive recommendations for the ingestion of KI from the SRC and will make his/her recommendation to the Officer-in-Charge (OIC) of the County. At this point the OIC advises all agencies to ingest KI. The County RO will also evaluate requests from county public safety officials who request an exception to worker exposure limits. 3 Criterion 2.b.1: Radiological Assessment and Protective Action Recommendations and Decisions:
Appropriate protective action recommendations are based on available information on plant conditions, field monitoring data and licensee and ORO dose projections, as well as knowledge of on-site and off-site envirownental conditions.
3 State of Wisconsin The SRC staff will evaluate information provided by the licensee on plant conditions and perform independent dose assessment and projections. These assessments and projections are based on plant conditions, field-monitoring data (provided by the State restricted area Field Teams through the FOC/MRL), and knowledge of on-site and off-site environmental conditions and monitoring plant conditions electronically via ERDS.
3 Field monitoring data will be provided via controller inject messages through the restricted area field teams to the Field Team Coordinator. The SRC will evaluate the data and make a protective action recommendation to the OIC. Generally, early dose assessments performed by state dose assessment personnel will be based upon plant conditions obtained via telephone or facsimile from the affected plant. Later dose assessments will be developed based upon plant conditions and simulated field sampling results.
The.State SRC will also use dedicated laptop computers for RASCAL dose I
assessment and plume modeling. GIS will be used for display of status maps. The SRC will evaluate the data and consider environmental conditions and make a protective action recommendation to the OIC.

After Action Report/lInprovement Plan Point Beach Nuclear Plant Final 10/19/2010 Criterion 2.b.2: Decisions for the Plume Phase of the Emergency: A decision-making process involving consideration of appropriate factors and necessaly coordination is used to make protective action decisions for the general public, including the recommendation for the use of KI, if ORO policy.
State of Wisconsin As a "home rule" state, the Governor or his/her designee is informed of the protective action recommendations (PARs), but the decision to change the PAR to a Protective Action Decision (PAD) ultimately resides with the chief elected official (or his/her designee) in the county, who may modify PARs based on local needs or considerations.
The state OIC, or an Operations designee, will contact the Kewaunee and Manitowoc County Emergency Management Directors and relay the PAR to ensure that the Counties have no events taking place that will prohibit implementation of the PAR.
Upon concurrence by both counties the PAR becomes a Protective Action Decision (PAD. The state OIC informs the Governor or his representive of the PAD. Then Kewaunee and Manitowoc County EOCs and the SEOC agree upon times to sound sirens and broadcast the appropriate EAS messages to inform the general public.
The Wisconsin Emergency Response Plan (WERP) directs that K1 is not.made available to the general public in Kewaunee and Manitowoc Counties.
Manitowoc County Manitowoc. County's Chief Elected Official or his/her designee will evaluate protective action recommendations from the SEOC and determine if those recommendations are appropriate for the local situation.
The decision by county officials to implement state protective action recommendations is coordinated between the SEOC and both risk counties.

Nursing Home decision makers have chosen to shelter their residents in place. KI doses are available in the county's supply for patients and staff. Distribution will be simulated if it is determined to issue KI.

Emergency workers will follow procedures in their dosimeter kit regarding radiological monitoring equipment use and exposure limits and will monitor and record dosimeter readings on the form provided.
Manitowoc County Sheriffs Department deputies assigned to work in the field and I
Manitowoc County Highway Commission workers assigned to the traffic/access control point will receive dosimeter kits and faux KI. They will be briefed by their shift supervisors on the day of the exercise at two locations: Patrol Squad Room, and/or the traffic control point, which will be determined by the scenario.
Coast Guard personnel responding in the field would be expected to have their own dosimetry. If they do not, they would be advised to report to the Reception Center to be issued a dosimeter kit and be briefed before reporting to the field.

The Manitowoc County Health Officer's representative, in consultation with Wisconsin Department of Health Services WI-DHS, the County's RO, and OIC will advise emergency workers, through their respective department supervisors, when they should take the simulated KI.
Prior to take-off the airplane crew is given a briefing on the use of dosimetry and KI.
Kewaunee County Only county emergency workers entering the restricted area will use dosimetry kit containing KI, instructions, and a form for recording KI ingestion. Some kits may be pre-distributed. These kits will be issued from and returned to the County Annex Building, before assuming/completing shift duties. The Annex Nurse will give a briefing to a Law Enforcement officer, public works personnel assigned to the TACP, and a bus driver on the use of KI and dosimetry. PAD. The counties review the message and content and approve it "as is," or modify the message to reflect local weather, road construction, or other conditions prior to its release. Once the message of the teR2s agreed upon by the state and the counties (the I

An aircraft mounted public address system will be used to provide back-up to the primary warning system, if weather permits. It is also used to warn boaters on Lake Michigan. The aircraft mounted public address system will be demonstrated unless adverse weather conditions exist. The staff operating the aircraft will be available for evaluator interview.
Kewaunee County Kewaunee County will growl its sirens in accordance with procedures. The county will transmit a pre-scripted message on WSRG radio in Door County, Wisconsin.
WSRG does operate 24\\7, but it is not manned between the hours of 8pm to 4:30am.
The county has a plan to contact WSRG's on-call person at the Alert, who would then go to the station. The County will then transmit a pre-scripted EAS Message and Special News Broadcast Message to the station.
The station will simulate the broadcast of both messages. Kewaunee County will fax a hard copy of the message to WSRG. Kewaunee County will demonstrate this procedure once for evaluation purposes. Siren testing data is submitted to FEMA annually with the Annual Letter of Certification packet. WSRG-Radio Station will have staff available for interview during the exercise. A county escort will be available upon request.
Criterion 5.a.2: Activation of the Prompt Alert and Notification System (Fast Breaking):
Activities associated with primary alerting and notification of the public are completed within 15 minutes of verified notification from the utility of an emergency situation requiring urgent action (fast breaking situation). The initial instructional message to the public must include:

is a procedure in place for officers from the Sheriffs Department to drive through Area A and Area B to notify residents in the event of siren activation. The back-up alerting will be demonstrated for both Area A and Area B.

The State of Wisconsin PIOs in the JIC will coordinate with the State EOC and PIOs from the counties, and the utility to ensure the media is briefed in a clear, accurate, and timely manner. Procedures for early notification of the media will be demonstrated in the State EOC prior to the activation of the JIC. Public inquiry calls will be handled by the Point Beach Call Center at the JIC, as stated in the State of Wisconsin/Point Beach Letter of Agreement. The Call Center will be staffed by Point Beach personnel.
A media hotline will be established, which will allow the media to call directly into the JIC PIO room rather than calling the Public Inquiry Hotline. This media phone line will allow members of the media to contact a JIC PIO staff person who will take the media calls and disseminate the call to the proper county, state, or utility PIO representative.

State of Wisconsin The MS-I hospital drill will be held out-of-sequence on the day of the exercise at the Aurora Bay Care Hospital in Green Bay, WI. The hospital portion of the drill will be a continuation of the Kewaunee County ambulance drill. Set-up for the drill will be at the Luxemburg Rescue Squad parking lot. The transportation portion of the drill should transition to the hospital portion. The hospital will demonstrate setting up the Radiological Emergency area, providing appropriate medical care to the patient, and monitoring and decontamination of the patient.
Manitowoc County I
Manitowoc Fire Department EMS personnel will demonstrate the packaging and preparation for transport of a potentially contaminated medical victim to the Hospital.
The ambulance will also demonstrate communication with the hospital per its SOP.
Monitoring sick or injured individuals for contamination is not normally done at the reception center or at an incident scene in the field, since a full survey is always required at the hospital.
Therefore, ambulance crews are not issued survey instruments, but will be issued TLDs and DRDs in accordance with the plan.
Kewaunee County The transportation section of the drill will be demonstrated out of sequence beginning at the Luxemburg Rescue Squad parking lot (in Luxemburg). Luxemburg Rescue personnel will demonstrate the packaging and preparation for transport of a potentially contaminated medical victim. Ambulance personnel will make a report to the Medical Center, they will then transported the patient to Aurora Bay Care Hospital in Green Bay, WI.

NARRATIVE SCENARIO This simulated radiological accident begins as a member of the general public arrives at the reception center in Manitowoc. The individual has been evacuated from the area around the Point Beach Nuclear Plant due to an accident and release of radioactive materials. While driving to the reception center, via a locally known shortcut, the person swerves to avoid a deer in a wooded area and drives off the road.
While exiting the car in the ditch the individual trips on a downed tree branch, falling and striking their forehead on a rock resulting in a severe laceration and severely sprained ankle. A short time later, while the victim lay dazed next to their car, another I
vehicle comes along and stops to help. While assisting the injured person, both are exposed to the passing plume. Eventually they leave, driving the injured person to the reception center.
The driver tells reception center staff that the car and passengers may have been exposed to the radioactive plume resulting in potential contamination of both.
The ambulance crew stationed at the reception center treats the victim, implements contamination control measures, and loads the individual into the ambulance for I
transport to the hospital. Reception center staff is trained not to survey evacuees requiring immediate medical attention, since thorough surveys will be performed at the receiving hospital.

IV.
NARRATIVE SCENARIO This drill begins with a simulated radiological release at the Point Beach Nuclear Plant. A member of the general population experiences a flat tire while attempting to evacuate the area. While changing the tire, the victim's car slips off of the jack and the jack strikes the victim in the left leg breaking the left fibula.
While dealing with this situation the victim and their passenger believe they were exposed to the plume.
The passenger finishes changing the0tre and drives the victim to Luxemburg to receive medial treatment at the rescue station at the direction of 911 operators.
The driver informs the rescue personnel that they and the car are most likely radioactively contaminated from the plume.

After Action Report/Improvement Plan Point Beach Nuclear Plant The rescue squad stationed in Luxemburg treats the victim, implements contamination control measures, and properly loads the individual into the ambulance for transport to the hospital. Rescue squad staff are trained not to survey evacuees requiring immediate medical attention, since thorough surveys will be performed at the receiving hospital. General medical conditions of the victim can be found in Section V below.
The ambulance crew directs the remaining uninjured passenger to proceed to the reception center in Algoma where both the car and the passenger can be screened for contamination and decontaminated if necessary.
Rescue squad personnel medically evaluate the patient and prepare the victim for transport to Aurora BayCare Medical Center in Green Bay. Aurora BayCare Medical Center is notified of the victim's condition and estimated time of arrival, prepares the Radiation Emergency Area (REA), and notifies the appropriate staff according to procedures. The ambulance crew continues patient care and transports the patient to Aurora BayCare Medical Center.
The hospital emergency staff meets the ambulance upon arrival. An initial report is received and the patient is transferred to the REA. The hospital staff evaluates the patient and initiates appropriate treatment. The hospital radiation safety personnel perform a radiological survey of the patient. Bioassay samples are taken from the wound area and intact skin areas. Decontamination efforts are performed until surveys indicate acceptable readings on previously contaminated areas.
The patient is surveyed prior to transfer out of the REA. Appropriate exit procedures for the patient and attending personnel are performed. Surveys of the ambulance crew and vehicle are performed prior to releasing the crew and vehicle.
