6-INDIAN POINT 2 - (UPDATE) ALERT DECLARED DUE TO EXPLOSION IN THE 21 MAIN TRANSFORMER -) PNO-1-10-006 / EN 46400 Update 15 Nov: The licensee has preliminarily determined that a failed bushing (internal portion) in the 'B' phase caused the transformer failure. During evaluation of the event sequence, the licensee also determined that one of twelve exhaust fan ventilation dampers did not close as expected upon transformer deluge system actuation. The dampers provide fire/smoke protection to the 480V bus room from the transformer yard. These dampers are operated by fusible links and operate to close the dampers based upon deluge system actuation 2
&o'room temperature setpoints. The licensee is investigating whether the damper was mechanically bound or if
'the fusible links failed to function correctly. Notwithstanding the damper failure, no smoke, oil or water intrusion was reported in the 480V switchgear room during or after the event.
