- 4) EN 46400 ;-'INDIAN POINT 2 - ALERT DECLARED DUE TO EXPLOSION IN THE 21 MAIN TRANSFORMER See EN'Text. The following are notes from regional call: An equipment operator witnessed the explosion (was not injured) and reported that flames dissipated quickly'with the actuation of the deluge system. The fire brigaderesponded but did not have to fight an active fire.i The fault is suspected to originate from a failure in the B phase. The station auxiliary transformer tap changer (reported inoperable in EN) was repaired; rno voltage control issues were noted while the tap changer was inoperable.
The licensee may conduct an offsite notification due to oil (released during explosion) having reached the discharge canal Outside of Scope 2
