The ES-201 Form, Examination Security Agreement, is in the process of being signed off as personnel become available. The ES-201 Form will be sent at a later date as soon as all required signatures are obtained.

Enclosed you will find the following forms and materials for the NRC Written examination administered on 04-13-2010 at Farley Nuclear Plant.

1) Six exam answer sheets: 5 SRO and 1 RO for electronic scanning in LXR software (scantron sheets) and copies of the same.
2) 6 Exam cover sheets signed by Applicants.
3) 2 Preliminary grade Detailed reports.
4) Statistics on each question for each of the RO and SRO exams.
5) Written Exam seating chart.
6) 2 questions asked by applicants during exam with answers given.
7) Question 43 paperwork for consideration to be deleted. (7 pages)
8) Question 68 paperwork for consideration to accepted two answers as correct. (49 pages)
9) Student comments on other questions.
