Background

  • On March 15, following discovery of cracks on two control rod drive mechanism nozzles, NRC dispatched an inspector to assist the resident inspectors
  • On March 1717, response upgraded to a special inspection team and supplemented with additional inspectors onsite
  • NRC focus during special inspection:

- evaluate previous licensee activities with respect to the reactor head; and

- evaluate its suitability for continued service 7

- Discuss the Outage examinations and the examination that lead to discovery of the reactor pressure vessel head nozzle degradation and leakage

- Discuss nozzle repairs and additional inspections completed to ensure Reactor Vessel Head integrity 8

- Discuss Conclusions reached and the Basis of Acceptability for placing the Reactor Vessel Head back into service

- Discuss the Basis for the Duration of Operation until next Reactor Vessel Head Inspection
