Dear Mr. Schimmel:
This letter refers to an investigation conducted by the U.S. Nuclear Regulatory Commission (NRC),
Office of Investigations (OI) to determine whether a supervisor and a staff member deliberately violated regulatory requirements and plant procedures by failing to report an incident on April 16, 2009. Based on the evidence developed, we did not substantiate the allegations.
