Dear Sir or Madam:

Crane Nuclear, Inc. has become aware of a deficient fillet weld that was identified by a customer and the subsequent review by Crane of all weld records revealed that the inspection of some leak off pipes / vent was not documented.

Crane Nuclear, Inc. has reviewed design and manufacturing records for all orders which had this type of weld. From this review we have determined that only a small number of orders did not have proper documentation of the weld inspection to support design compliance. Crane has sent I 0CFR21 notification letters on this issue to the following customers:

US Nuclear Regulatory Commission I OCFR2I Notification of a Potential Weld Defect March 25, 2010 - Page 2 Crane has recommended that the owners perform an inspection of the weld to determine its compliance to the design requirement for the fillet weld sizes identified above. Crane is interested in correcting our records and we have asked that the utility to provide information relative to inspections performed.

Crane has submitted via overnight mail a letter to each of the affected customers including the utility that notified Crane initially of the potentially deficient weld.

The immediate corrective measures taken to preclude recurrence of this deficiency was to verbally discuss this issue with all quality assurance, quality inspection and engineering personnel and a more formal process is underway through our corrective action program to prevent recurrence.
