CONSENT AND HEARING WAIVER FORM The Tennessee Valley Authority (TVA) hereby agrees to comply with the terms and conditions of the Confirmatory Order to be issued relating to enforcement action NOS. EA-09-009 and EA-09-203 which will be immediately effective upon its issuance.
