INTEGRATED INSPECTION REPORT 05000482/2009004
Dear Mr. Muench:
This acknowledges receipt of your letter (WM 09-0065) dated December 9, 2009, in which you
disputed noncited violations NCV 05000482/2009004-03 Inadequate Evaluation of Emergency
Diesel Generator for Common Cause Failure in the Supporting Essential Service Water
System, and 05000482/2009004-06, Performing Prohibited Elective Maintenance on Safety
Bus NB02 Channel 4 during Emergency Diesel Generator Maintenance. Your letter also
provided information related to noncited violation NCV 05000482/2009004-07, Failure to
Report Conditions that Could Have Prevented Fulfillment of a Safety Function. We will review
the information you provided when assessing the disputed violations.
