2009 DISCHARGE MONITORING REPORT - QUALITY ASSURANCE (DMR-QA) STUDY 29 CORRECTIVE ACTIONS REPORTS Please find enclosed the corrective action reports for the 2009 DMR-QA Study 29 Laboratory Performance Evaluation obtained by TVA Sequoyah Nuclear Plant (SQN) and supporting laboratories as required by NPDES Permit TN0026450. Investigations concluded that both "Not Acceptable" results were isolated incidents due to the lack of obvious causes of failure. Remedial samples have also been reanalyzed and were found to be "Acceptable." We were unable to determine an obvious cause of failure. ESC has since analyzed another PT for TSS with acceptable preliminary results. We believe this to be an isolated incident and have evaluated all QC criteria concerning this matter. The laboratory has performed all appropriate corrective actions and made changes where necessary.

December 3, 2009 SEQUOYAH NUCLEAR PLANT DMR-QA 29 TOTAL RESIDUAL CHLORINE "NOT ACCEPTABLE" RESULTS The Sequoyah Nuclear Plant (SQN) Chemistry QA/QC program owner has performed an investigation into the DMR-QA 29 sample concerning Total Residual Chlorine (TRC). SQN was unable to determine an obvious cause of the "not acceptable" results. SQN has since analyzed another sample for TRC with acceptable results. SQN believes this to be an isolated incident and has evaluated all QA/QC criteria concerning this matter. SQN has performed all appropriate corrective actions regarding this "not acceptable" result.
