SUMMARY

OF RESULTS The qualification program consists of the shunt trip, close coil, and undervoltage trip (UV) for the Masterpact low voltage circuit breakers. Electromagnetic Interference/Radio Frequency Interference (EMI/RFI) qualification was be performed by testing in accordance with EPRI TR-102323, revision 3 [2] and the NLI qualification plan [3].

The acceptance criteria were met. One anomaly was documented, which does not impact the results of the testing. The anomaly is summarized in section 4.2.7 and is contained in Attachment A. Based on the results of this testing, the components are qualified for the specified service conditions.

This testing applies to the shunt trip, UV, and close coils used in the Masterpact NT and NW circuit breakers. Additional details are presented in section 2.2.

The baseline functional testing was performed at the NLI facility in Fort Worth, Texas. The EMI/RFI testing was performed at NEMKO, an approved NLI vendor.

2.0 EQUIPMENT IDENTIFICATION 2.1 Test Specimen The test specimen consisted'of a Masterpact NW breaker with the following accessories installed:

  • Shunt trip (nominal rating of 125vdc/120vac): Square D p/n S33812.
  • Undervoltage trip (nominal rating of 125vdc/120vac): Square D p/n S33821.

The testing also applies to the following Masterpact components (see the traceability evaluation in section 2.2):

  • Masterpact NW breaker: close coil.
  • Masterpact NT breaker: shunt trip, UV trip, and close coil.

Additional information on the operation of the coils is presented in section 2.3. Rev. 1 2.2 Traceability of the Test Specimen to the Production Units This qualification testing applies to the following components:

  • Shunt trip and close coil (nominal rating of 125vdc/120vac):

o NW breaker: Square D p/n S33812.

o NT breaker: Square D p/n S48493.

  • Undervoltage trip (nominal rating of 125vdc/120vac):

o NWbreaker: Square Dp/n S33821.

" NT breaker: Square D p/n S48503.

  • All of the coils use the same housing, mounting, circuit board, microcontroller, and firmware. The mechanical assembly is different as follows; o The close coil and shunt trip are the same part and part number. They extend the plunger when power is applied and spring return the plunger when power is removed (or voltage is lowered below the setpoint).

The functional requirements on the breaker is the same. The coils are normally de-energized. They are energized for a short duration, which extends the plunger and hits the latch (trip latch for the shunt trip and close latch for the close coil).

o The UV coils retract the plunger when power is applied and spring return the plunger when power is removed (or voltage is lowered below the setpoint).

  • Voltage rating: The coils are dual AC/DC rated. Testing was performed at 125vdc nominal. The testing applies to the AC application of the coils as follows:

o The power to the coils will not impact the emissions or susceptibility of the coils.

  • NT vs. NW part number:

o The coils are the same except for the wire leads and the connectors at the end of the leads. This does not impact the emissions or susceptibility of the coils.

" Close coil: The close coil is the same part as the shunt trip and performs the same function (extend the plunger when power is applied and spring return the plunger when power is removed). The testing of the shunt trip applies to the close coil as follows:

o (conducted emissions and susceptibility, fast transients, surge): The testing on the shunt trip was performed on the wiring directly to the shunt trip. This is the same configuration as the close coil.

o (radiated emissions and susceptibility): The close coils and shunt trip are adjacent to each other in the breaker.

2.3 Safety Function See below for details on the operation of the coils.

The safety functions of the coils are as follows: Rev. I

  • Shunt trip: Trip the breaker when control power is applied to the shunt. Not inadvertently trip the breaker when control power is not applied to the shunt.

" Undervoltage: Allow the breaker to close and stay closed when control voltage is applied to the UV. Trip the breaker when the control voltage is reduced below the trip value or removed.

" Close coil: Close the breaker when power is applied to the coil. Not inadvertently close the breaker when power is not applied to the coil.

I. a Masterpact EMI/RFI Qualification Report QR-042181-5, Rev. 1 Nuclear Logistics, Inc. Page 3 The EMI/RFI testing addresses all of the operating conditions and potential failure modes of the coils due to the EMI/RFI conditions, as summarized below:

1. All of the coils (UV, shunt, close) use the same microcontroller, firmware, and circuit board. The only difference is the configuration of the mechanical spring. The spring is a mechanical part and is not impacted by the EMI/RFI testing.
a. Note that the shunt and close coils are the same part number.
2. The safety function of the coils is as follows:
a. Maintain the specified position (energized or de-energized) and not inadvertently change the position of the plunger due to EMI/RFI conditions.
b. Change the position of the plunger when the proper voltage signal is applied (shunt trip, close coils) or removed (UV coil).
3. The testing was performed with the UV coil energized and the shunt trip de-energized.

All susceptibility test were performedon both pairs of lines.

4. The specific safety functions of the coils were demonstrated as follows:
a. No inadvertent operation was demonstrated as follows:
i. The testing documented that the UV would not inadvertently drop out and trip and the breaker and the shunt trip would not inadvertently pick up and trip the breaker.

ii. Testing of the shunt trip in the de-energized position, per above, documents that the close coil will not inadvertently actuate and inadvertently close the breaker. The two coils are the same part number.

b. Proper operation of the coils due to the changes in voltage was demonstrated as follows:
i. Proper operation of the shunt trip/close coils is for them to change state (energize coil) when voltage is applied. This is the same condition as the tested condition of the UV coil (energized coil). Testing of the UV coils energized demonstrated that voltage can be applied to the shunt trip/close coils during the EMI/RFI condition and the coil would properly energize.

Misoperation of the energized UV due to the EMI/RFI conditions would cause the mechanical spring return to extend the plunger and trip the breaker. This is a mechanical function that is not impacted by the EMI/RFI conditions. The proper operation of the UV coils is to de-energized and allow the spring return to extend the plunger. There is not a credible mechanism for the EMI/RFI conditions to prevent the spring return from extending the plunger to trip the breaker. Rev. I

" The coil activation is electronic. There are no 'relays.

  • The writeup below is for the shunt trip. The UV coil uses the same logic, just different mechanical operation-of the coil.

The following additional information is provided:

  • The electronics measure the voltage and coil activation current. The measured voltage values are compared to coil activation and inhibit table values to determine if coil energization is required.
  • The electronics also regulate the current draw of the coils to prevent overheating.

Temperature buildup is controlled by limiting the maintain current to the coils. The temperature of the coil wires must be less than 150'C.

  • The temperature around the electronic board must remain below 120'C.

The electronic switch monitors the input voltage and when the input voltage exceeds the preset threshold, the activation coil is energized to operate the plunger.

After the plunger is activated the electronic switch shuts off the activation coil and energizes the maintenance coil to maintain the plunger in the activated position.

If the input voltage is below the preset operating level of the device, the electronic switch will not energize either coil and the only current draw by the device will be to keep the electronic switch powered. Rev. I DC Power Activation Maintenance Supply Electronic switch Coil Coil AC Rev. I

2.4 Interfaces/Test Configuration The interfaces/iest configuration was as follows during the testing:

" The shunt trip and UV were installed in a Masterpact breaker using the standard Square D mounting. The standard Square D wiring from the shunt trip and UV to the secondary disconnects was used.

  • The shunt trip and UV were wired out from the secondary disconnects for application of the control voltage using standard #14 SIS wire. No conduit was used.
  • The Masterpact circuit breaker was sitting on the test bench with no enclosure. This is conservative, since the normal installation is in metal switchgear.
  • The Masterpact circuit breaker was grounded per the normal breaker ground.
  • No primary power was applied to the breaker. This is acceptable as follows:

o The shunt trip and UV receive all power from the control power, which is separate from the primary power. There is no electrical interface between the primary power and the control power.

" The test specimen was tested in the following configuration:

o Pick up the UV and hold at a degraded voltage 105vdc.

o The breaker was closed.

o The shunt trip was de-energized.

  • The testing was performed with the breaker in the normal configuration. The testing verified whether the susceptibility testing would impact the operation/inadvertently change to state of an energized coil and a de-energized coil.

" Radiated emissions and susceptibility testing was performed at the front of the breaker.

These components are mounted near the front of the breaker.

3.0 PLANT CONDITIONS 3.1 Service Conditions The service conditions are specified as follows:

  • EMI/RFI service conditions per EPRI TR-102323, revision 3.

3.2 Functional Parameters The rated control voltages for the shunt trip and UV are as follows (dual rated for AC and DC):

  • Shunt trip (trip): 125vdc nominal (88-137vdc).

" UV: pickup 100-140vdc, hold-in 88-140vdc, drop out 42-87vdc.

3.3 Margin The EMI/RFI levels specified in EPRI TR-102323, revision 3 are conservative. No additional margin was added.

4.1 Baseline Functional Testing Baseline functional testing was performed to verify proper operation of the test specimen.

Testing and acceptance criteria were in accordance with baseline functional test BFT-042181 1. The acceptance criteria were met. The data sheets are contain in Attachment A.

4.2 EMI/RFI Testing The EMI/RFI testing was performed as specified in this section. The test details are contained in the NEMKO report in Attachment A.

4.2.1 Acceptance Criteria Acceptance Criteria The test specimen met the requirements in Section 4.2.2 through 4.2.11. The specific acceptance criteria are as follows:

  • Emissions levels were lower than the Allowable Equipment Levels in EPRI TR-102323, revision 3.
  • The circuit breaker did not change state during the susceptibility testing:

o UV does not drop out and trip the circuit breaker.

o The shunt trip does not pick up and trip the breaker.

4.2.2 Low Frequency Conducted Susceptibility (30 Hz to 150 kHz)

Testing was performed in accordance with MIL-STD-461E, CS101. The test limits were per EPRI TR-102323, revision 3, figure 5-1, Curve 1.

The signal was injected into the following lines:

" Undervoltage.

The test specimen met the acceptance criteria.

Testing was performed in accordance with MIL-STD-461 E, CS 114, Curve 3 (also see EPRI TR-102323, Figure 5-2).

The signal was injected into the following lines:

  • Undervoltage.

The test specimen met the acceptance criteria.

4.2.4 Low Frequency Radiated Magnetic Field Susceptibility (30 Hz to 100 kHz)

Testing was performed in accordance with MIL-STD-461 E, RS 101 Army applications (also see EPRI TR-102323, Figure 5-3).

The testing was performed at the front of the breaker.

The test specimen met the acceptancecriteria.

4.2.5 High Frequency Radiated Electric Field Susceptibility (30 MHz to 10 GHz)

The testing was performed in accordance with MIL-STD-461E, RS 103 for Navy Ship limits.

The testing was performed at the front of the breaker.

The test specimen met the acceptance criteria.

4.2.6 Low Frequency Conducted Emissions (120 Hz to 10 kHz)

Testing was performed in accordance with MIL-STD-461E, CE101. The acceptance criteria are that the emissions are below EPRI TR-102323, Figure 5-4, curve 1.

The emissions were measured on the following lines:

  • Undervoltage.

The test specimen met the acceptance criteria.

Testing was performed in accordance with MIL-STD-461E, CE102. The acceptance criteria are that the emissions are below EPRI TR-102323, Figure 5-5. The acceptance criteria can be relaxed per the note on Figure 5-5.

The emissions were measured on the following lines:

" Undervoltage.

The test specimen met the acceptance criteria.

Anomaly 042181-5-1: Per the NEMKO report in Attachment A, the emissions exceeded the allowable. The testing is considered acceptable, as follows:

  • Per the NEMKO report [page 13], the total emissions exceeded the allowable at certain frequencies. The emissions exceeded the allowable at certain frequencies by a small amount. The allowable emissions in R.G. 1.180 are slightly higher than the EPRI TR-102323 levels, and the emissions are below the R.G 1.180 levels. The emissions are considered acceptable.
  • The testing could not be performed on the shunt trip, since there'was no power to the coil. The testing on the UV documents the emissions are acceptable, since the same circuit board and microcontroller are used in the UV and shunt trip.

4.2.8 Low Frequency Radiated Emissions (30 Hz to 100 kHz)

This testing was not performed. The equipment is not a source of large magnetic fields.

4.2.9 High Frequency Radiated Emissions (2 MHz to 10 GHz)

Testing was performed in accordance with MIL-STD-46 1E, RE 102. The acceptance criteria are that the emissions are below EPRI TR-102323, Figure 5-7.

The radiated emissions were measured at the front of the circuit breaker.

The test specimen met the acceptance criteria.

Masterpact EMI/RFI Qualification Report QR-042181-5, Rev. 1 Nuclear Logistics, Inc. Page 9 4.2.10 Electrical Fast Transients/Burst Testing was performed in accordance with MIL-STD-461, CS 115. The test voltages were as specified below. The pulse shape and repetition were per the MIL-STD.

The signal was injected into the following lines:

  • Undervoltage.

Voltage = +/-2kV. Used a capacitive clamp.

The test specimen met the acceptance criteria.

4.2.11 Surge Withstand Capability This testing was performed last.

Both of the following tests were performed:

" IEC EN 61000-4-5, Level 3.

" IEC EN 61000-4-12, Level 3.

The test limits were +/- 2Kv. The pulse shape, repetition, and burst durations were per the identified IEC standards.

Testing was performed on the following lines:

" Undervoltage.

Testing was performed line/line and line/ground.

The test specimen met the acceptance criteria.

4.2.12 Electrostatic Discharge Susceptibility Per EPRI TR-102323, ESD is not considered a common mode failure mechanism for safety related equipment and the test is considered optional. This test was not performed.

Note that after installation, there are no personnel interfaces with these devices.

4.3 Baseline Functional Testing Baseline functional testing was performed to verify proper operation of the test specimen.

Testing and acceptance criteria were in accordance with baseline functional test BFT-042181 1. The acceptance criteria were met. The data sheets are contain in Attachment A.

5.2 Modifications No modifications were made to the test specimen.

6.0 QUALITY ASSURANCE Project activities were performed in accordance with the NLI Quality Assurance Program which meets the requirements of 10CFR50 Appendix B, 10CFR21 and ASME NQA-1 [1].

NEMKO is an approved NLI vendor and has been audited for this EMI/RFI testing. Calibration of M&TE was included in the audit scope.

7.0 MEASUREMENT & TEST EQUIPMENT Measurement & Test Equipment which are used are controlled by the NLI M&TE program (procedure NLI-QUAL-05, latest revision). The test data sheets identify the M&TE which was used. The calibration of M&TE is traceable to NIST or equivalent standards. The accuracy of the calibrated M&TE that was used is appropriate for the testing that was preformed.

NEMKO is an approved NLI vendor and has been audited for this EMI/RFI testing. Calibration of M&TE was included in the audit scope.

Test Method:

1. Applicable Test Standard: MIL-STD-461E.
2. For shielded room measurements electrically bond the rod antenna counterpoise to the ground plane using a solid metal sheet the same width as the counterpoise. The maximum DC resistance between the counterpoise and the ground plane shall be 2.5 milliohms. For bench top setups using a metallic ground plane, bond the counterpoise to this ground plane. Otherwise, bond the counterpoise to the floor ground plane. For measurements outside a shielded enclosure, electrically bond the counterpoise to earth ground.
3. Antenna Positioning.

(a) Determine the test setup boundary of the EUT and associated cabling for use in positioning of antennas.

(b) Use the physical reference points on the antennas shown in Figure RE102-6 for measuring heights of the antennas and distances of the antennas from the test setup boundary.

1. Position antennas 1 meter from the front edge of the test setup boundary for all setups.
2. Position antennas other than the 104 cm rod antenna 120 cm above the floor ground plane.
3. Insure that no part of any antenna is closer than 1 meter from the walls and 0.5 meter from the ceiling of the shielded enclosure.
4. For test setups using bench tops, additional positioning requirements for the rod antenna and
a. Distance above the bench ground plane is shown in Figure RE102-6.
5. For free standing setups, electrically bond and mount the 104 cm rod antenna matching
a. Network to the floor ground plane without a separate counterpoise.
6. The number of required antenna positions depends on the size of the test setup boundary and the number of enclosures included in the setup.

(1) Turn on the EUT and allow sufficient time for stabilization.

(2) Select test frequencies as follows:

(a) Locate the loop sensor 5 cm from the EUT face or electrical interface connector being probed. Orient the plane of the loop sensor parallel to the EUT faces and parallel to the axis of connectors.

(b) Supply the loop with sufficient current to produce magnetic field strengths at least 10 dB greater than the applicable limit but not to exceed 15 amps (183 dBpT).

(c) Scan the applicable frequency range. Scan rates up to 3 times faster than the rates specified in Table III are acceptable.

(d) If susceptibility is noted, select no less than three test frequencies per octave at those frequencies where the maximum indications of susceptibility are present.

(e) Reposition the loop successively to a location in each 30 by 30 cm area on each face of the EUT and at each electrical interface connector, and repeat 5.18.3.4c(2)(c) and 5.18.3.4c(2)(d) to determine locations and frequencies of susceptibility.

(f) From the total frequency data where susceptibility was noted in 5.18.3.4c(2)(c) through 5.18.3.4c(2)(e), select three frequencies per octave over the applicable frequency range.

(3) At each frequency determined in 5.18.3.4c(2)(f), apply a current to the radiating loop that corresponds to the applicable limit. Move the loop to search for possible locations of susceptibility with particular attention given to the locations determined in 5.18.3.4c(2)(e) while maintaining the loop 5 cm from the EUT surface or connector. Verify that susceptibility is not present.

Page 1 1.0 SCOPE The qualification program consists of the shunt trip and undervoltage trip (UV) for the Masterpact low voltage circuit breakers. Electromagnetic Interference/Radio Frequency Interference (EMI/RFI) qualification will be performed by testing in accordance with EPRI TR-102323, revision 3 [2].

Note: Based on NLI's experience, most nuclear power plants require testing per EPRI TR-102323, revision 3. Therefore, these test requirements are used as the basis for this test plan instead of alternate industry and regulatory documents.

2.0 EQUIPMENT IDENTIFICATION 2.1 Test Specimen The test specimen will consist of a Masterpact NT or NW breaker with the following accessories installed:

  • Shunt trip (nominal rating of 125vdc):

o NW breaker: Square D p/n S33812.

o NT breaker: Square D p/n S48493.

  • Undervoltage trip(nominal rating of 125vdc):

o NW breaker: Square D p/n S33821.

o NT breaker: Square D p/n S48503.

The electrical and mechanical configuration of the shunt trips for the NT and NW breakers are the same. The difference is the length of the coil wires and the wire connectors, which are different for the two breakers. Testing of one configuration is applicable to the other.

2.2 Traceability of the Test Specimen to the Production Units Traceability of the test specimen to the production units will be performed. The following methodology will be used to document the traceability:

  • The test specimens and,the production units will be the same manufacturer/model.

Testing of the NT or NW shunt and UV trips is applicable to the other breaker.

2.3 Safety Function The safety functions are as follows:

  • Shunt trip: Trip the breaker when control power is applied to the shunt. Not inadvertently trip the breaker when control power is not applied to the shunt.

" Undervoltage: Allow the breaker to close and stay closed when control voltage is applied to the UV. Trip the breaker when the control voltage is reduced below the trip value or removed.

2.4 Interfaces/Test Configuration

" The shunt trip and UV will be installed in a Masterpact breaker using the standard Square D mounting. The standard Square D wiring from the shunt trip and UV to the secondary disconnects will be used.

" The shunt trip and UV will be wired out from the secondary disconnects for application of the control voltage using standard #14 SIS wire. No conduit will be used.

" The Masterpact circuit breaker will be sitting on the test bench with no enclosure. This is conservative, since the normal installation is in metal switchgear.

" The Masterpact circuit breaker will be grounded per the normal breaker ground.

" No primary power will be applied to the breaker. This is acceptable as follows:

o The shunt trip and UV receive all power from the control power, which is separate from the primary power. There is no electrical interface between the primary power and the control power.

  • The test specimen will be tested in the following configuration:

o Pick up the UV and hold at 105vdc.

o The breaker will be closed.

" Radiated emissions and susceptibility testing will be performed at the front of the breaker. These components are mounted near the front of the breaker.

Document the breaker configuration during the susceptibility testing per the data sheet in Attachment A.

3.0 PLANT CONDITIONS 3.1 Service Conditions The service conditions are specified as follows:

  • EMI/RFI service conditions per EPRI TR-102323, revision 3.

3.2 Functional Parameters The rated control voltages for the shunt trip and UV are as follows (dual rated for AC and DC):

" Shunt trip (trip): 125vdc nominal (88-137vdc).

  • UV: pickup 100-140vdc, hold-in 88-140vdc, drop out 42-87vdc.

3.3 Margin The EMI/RFI levels specified in EPRI TR-102323, revision 3 are conservative. No additional margin will be added.

4.1 Baseline Functional Testing Baseline functional testing will be performed to verify proper operation of the test specimen.

Testing and acceptance criteria are in accordance with baseline functional test BFT-042181-5-1, which is contained in Attachment A.

4.2 EMI/RFI Testing The EMI/RFI testing will be performed as specified in this section.

4.2.1 Acceptance Criteria Acceptance Criteria The test specimen shall meet or exceed the requirements in Section 4.2.2 through 4.2.12. The specific acceptance criteria are as follows:

  • Emissions levels shall be lower than the Allowable Equipment Levels in EPRI TR- 102323, revision 3.

The signal will be injected into the following lines:

  • Undervoltage.

4.2.3 High Frequency Conducted Susceptibility (10 kHz to 200 MHz)

Testing will be performed in accordance with MIL-STD-461E, CS1 14, Curve 3 (also see EPRI TR-102323, Figure 5-2).

The testing will be at the front of the breaker.

4.2.5 High Frequency Radiated Electric Field Susceptibility (30 MHz to 10 GHz)

The testing will be performed in accordance with MIL-STD-461 E, RS 103 for Navy Ship limits.

The testing will be at the front of the breaker.

4.2.6 Low Frequency Conducted Emissions (120 Hz to 10 kHz)

Testing will be performed in accordance with MIL-STD-46 1E, CE 101.

The acceptance criteria are that the emissions are below EPRI TR-102323, Figure 5-4, curve 1.

The emissions will be measured on the following lines:

, Shunt trip.

  • Undervoltage.

4.2.7 High Frequency Conducted Emissions (10 kHz to 10 MHz)

Testing will be performed in accordance with MIL-STD-461 E, CE 102.

The acceptance criteria are that the emissions are below EPRI TR-102323, Figure 5-5. The acceptance criteria can be relaxed per the note on Figure 5-5.

The emissions will be measured on the following lines:

  • Undervoltage.

This testing will not be performed. The equipment is not a source of large magnetic fields.

4.2.9 High Frequency Radiated Emissions (2 MHz to 10 GHz)

Testing will be performed in accordance with MIL-STD-461 E, RE 102.

The acceptance criteria are that the emissions are below EPRI TR-102323, Figure 5-7.

The radiated emissions will be measured at the front of the circuit breaker.

4.2.10 Electrical Fast Transients/Burst Testing will be performed in accordance with MIL-STD-461, CS 115. The test voltages will be as specified below. The pulse shape and repetition will be per the MIL-STD.

The signal will be injected into the following lines:

" Shunt trip.

  • Undervoltage.

o Test both legs. Apply directly to the wires.

o Voltage = +/-2kV. Use capacitive clamp.

If the test specimens fails the testing, retest at +/-1kV.

4.2.11 Surge Withstand Capability This testing will be performed last. This testing is for information.

Both of the following tests will be performed:

  • IEC EN 61000-4-5, Level 3.
  • IEC EN 61000-4-12, Level 3.

0 Note: As an alternative, testing can be performed per IEEE C62.41.

The test limits will be +/- 2Kv. The pulse shape, repetition, and burst durations will be per the identified IEC standards.

Testing will be performed on the following lines:

  • Undervoltage.

Acceptance Criteria: The test specimen shall not incur any permanent damage from the application of surge excitations. Restoration of device operation following application of the above excitation shall not require any component replacement and shall require, as a maximum, the manual initiation of the device restart procedure.

Page 6 4.2.12 Electrostatic Discharge Susceptibility Per EPRI TR-102323, ESD is not considered a common mode failure mechanism for safety related equipment and the test is considered optional. This test will not be performed.

Note that after installation, there are no personnel interfaces with these devices.

4.3 Baseline Functional Testing Baseline functional testing will be performed to verify proper operation of the test specimen.

Testing and acceptance criteria are in accordance with baseline functional test BFT-042181-5-1, which is contained in Attachment A.

5.0 EVALUATION OF RESULTS 5.1 Anomalies Any anomalous behavior which is identified during any phase of testing will be documented and evaluated. The evaluation will determine whether the anomaly was due to test conditions or an unrelated cause such as improper test setup. If the anomaly was due to the test conditions, its impact on the safety function of the specimen will be determined. The anomaly disposition will be in one of the following manners based on engineering evaluation:

  • No impact on operational requirements or safety function.
  • Anomaly acceptable.
  • Retest.
  • Item not suitable for function/service conditions.

Anomalies which are due to unrelated causes will be identified as such, and the test will be continued.

Anomalies will be documented using the Test Anomaly Form 5.1.

5.2 Modifications Any modifications made to the test specimen during the testing sequence will be fully documented and evaluated. The impact on the qualification of each specimen will be determined.

If the modification has an impact on the qualification or the ability of each specimen to pass the qualification testing, a retest of the test specimen will be performed. If the modification has no impact on the qualification or the ability of the specimen to pass the qualification testing, a retest will not be required. Evaluation of the impact of any modifications will be based on each specimen's safety function and the test parameters.

6.0 QUALITY ASSURANCE Project activities will be performed in accordance with the NLI Quality Assurance Program which meets the requirements of 0OCFR50 Appendix B, 10CFR21 and ASME NQA-1 [1 ].

Page 7 7.0 MEASUREMENT & TEST EQUIPMENT Measurement & Test Equipment which are used will be controlled by the NLI M&TE program (procedure NLI-QUAL-05, latest revision). The Qualification Report will identify the M&TE which was used. A statement will be made in the Qualification Report as to whether the M&TE are traceable to NIST or equivalent standards. The accuracy of the calibrated M&TE that was used will be documented in the qualification report.

8.0 DOCUMENTATION A qualification report will be prepared which summarizes the testing, and presents the results.

The test data sheets, supporting calculations, and any other relevant data will be included in the report, where applicable.
