Enclosed for your information is the synopsis of the Nuclear Regulatory Commission (NRC)
Office of Investigations= (OI) completed report regarding whether a nuclear maintenance supervisor employed by the TVA willfully signed the RCIC procedure related to the over-speed test without verifying that the procedure had been completed, thus submitting incomplete and inaccurate information material to the NRC.
OI did not substantiate that a nuclear maintenance supervisor employed by the TVA willfully signed the RCIC procedure related to the over-speed test without verifying that the procedure had been completed, thus submitting incomplete and inaccurate information material to the NRC.
Accordingly, enforcement action is not warranted, and we plan no further actions with regard to this matter. Should you have any questions concerning this letter, please contact us.

Enclosure SYNOPSIS This investigation was initiated by the U.S. Nuclear Regulatory Commission (NRC), Office of Investigations (OI), Region II (RII), on March 2, 2009, to determine whether a nuclear maintenance supervisor employed by the Tennessee Valley Authority (TVA) willfully signed the Reactor Core Isolation Cooling (RCIC) procedure related to the "over-speed test" without verifying that the procedure had been completed, thus submitting incomplete and inaccurate information material to the NRC.
Based on the evidence developed during this investigation, OI:RII did not substantiate that a nuclear maintenance supervisor employed by the TVA willfully signed the RCIC procedure related to the "over-speed test" without verifying that the procedure had been completed, thus submitting incomplete and inaccurate information material to the NRC.
