Dear Sir/Madam:
This letter and attachment are provided per your telephone conversation with Mr. Gary Spicer on September 16, 2009 concerning a very brief bypass of the Ultraviolet Disinfection System by the final effluent from the sewage treatment facility. As noted, the circumstance was identified and addressed almost immediately. It is presumed that very little, if any, effluent reached the receiving water.

SUMMARY
Description of noncompliance (if an unauthorized discharge, include location, discharge route, and estimated volume): Effluent from Outfall 003 temporarily bypassed the UV disinfection system spillina to the around. Approximately 300 qallons of effluent that had received all appropriate treatment, i.e. activated sludge and clarification, except disinfection was released to the around near at the inlet to the UV unit. The overflow/bypass occurred as the result of a temoorary oluaaaqe of the UV system inlet Cause of noncompliance: Pluggaae of UV System inlet Duration:

Actions taken to mitigate adverse effects: Immediate area of release was disinfected with a sodium hypochlorite spray.
Actions taken to correct the problem and prevent recurrence:
Uoon discovery, the pluclaaaae of the inlet pipe was immediately removed which re-established the flow to the UV System. Personnel are investigating a lonG-term solution to this infrequent pluaaaae issue.
