Shana, I have done some research regarding the missing last 11 pages in Enclosure 5. John Hickman did not receive those pages also; however, the Document Control Desk did receive an entire copy, and the entire version has been placed into ADAMS.

We have entered this issue into our Corrective Action Program and are still trying to determine the cause and identify corrective actions to prevent recurrence. I believe it is not necessary to submit a supplement to the LAR because the "official" version was received correctly at the NRC and the entire version is in ADAMS. Therefore, I believe it is necessary to send the cc:'s the additional 11 pages without a formal submittal. Do you agree? Let me know when you return from leave.

I have a phone call with our CNO to discuss this matter on Tuesday at 8:30 am (PDT). If you could respond to me prior to that time (11:30 am EDT) I would appreciate it. Thank you.
