Dear Mr. Joyce:
The Region I Field Office, NRC Office of Investigations (OI), initiated an investigation (Case No.
1-2008-014) on December 11, 2007, to determine whether a control room senior reactor operator (SRO) at Hope Creek willfully violated site security procedures by tailgating out of the control room without using the card reader. Site procedures mandate the use of card readers for all doors that have a card reader so that positive access control is maintained for the control room, a vital area. Based on the evidence developed during the investigation, the NRC concluded that there was insufficient evidence to substantiate that an SRO tailgated out of the control room without using the card reader.
Although there was no first hand information of the alleged activity, OI obtained testimony from several reactor operators that they had heard rumors of occasions when an SRO had tailgated out of the control room to bypass a feature programmed into the security system to prevent all of the SROs from leaving the control room at the same time. However, none of the individuals interviewed had directly observed such an activity, or had first-hand knowledge of such an activity. OI also interviewed two SROs who were the subject of the above rumors. Both SROs were fully aware of the security requirements and stated that they had not tailgated to leave the control room. However, both SROs also stated that they were aware of circumstances that occurred during the refueling outage when they had left the control room for restroom breaks, at which time someone could have assumed that they had tailgated.
