1. Executive Summary On October 21 - 22, 2008, out-of-sequence Reception Center, Emergency Worker Monitoring and Decontamination and Medical Drills were conducted for the Waterford 3 Steam Electric Station (Waterford 3), located near New Orleans, Louisiana. Personnel from the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA) Region VI, evaluated all activities. The purpose of the drill was to assess the level of preparedness of local responders to react to a simulated radiological emergency at Waterford 3.

Personnel from the City of Paulina, West Jefferson Parish, the American Red Cross, and Waterford 3 participated in the drills. Evaluation Areas demonstrated included:

Emergency Operations Management, Protective Action Implementation, and Support Operations/Facilities. Cooperation, training and teamwork of all participants was evident during the drills, and DHS/FEMA wishes to acknowledge these efforts.

This report contains the final evaluation of the out-of-sequence drills. The participants demonstrated knowledge of their emergency response plans and procedures and adequately demonstrated them. There were no Deficiencies, one Area Requiring Corrective Action (ARCA), that was corrected on the spot, and one Plan Issue identified during the drills.

2. Introduction On December 7, 1979, the President directed the Federal Emergency Management Agency (FEMA) to assume the lead responsibility for all off site nuclear planning and response. FEMA's activities under the REP Program are conducted pursuant to 44 Code of Federal Regulations (CFR) Parts 350, 351 and 352. These regulations are a key element in the REP Program that was established following the Three Mile Island Nuclear Station accident in March 1979.

Rule 44 CFR 350 establishes the policies and procedures for the DHS/FEMA Region VI Office's initial and continued approval of tribal, state and local governments radiological emergency planning and preparedness for commercial nuclear power plants. This approval is contingent, in part, on state and local government participation in joint exercises with licensees.

FEMA's Region VI responsibilities in radiological emergency planning for fixed nuclear facilities include the following: Taking the lead in off-site emergency planning and in there view and evaluation of radiological emergency response plans (RERPs) and procedures developed by state and local governments; Determining whether such plans and procedures can be implemented on the basis of observation and evaluation of exercises of the plans and procedures conducted by state and local governments; Responding to requests by the U.S. Nuclear Regulatory Commission (NRC) pursuant to the Memorandum of Understanding between the NRC and FEMA dated June 17, 1993 (Federal Register, Vol. 58, No. 176, September 14, 1993); and Coordinating the activities of Federal agencies with responsibilities in the radiological emergency planning process:

Representatives of these agencies serve on the Regional Assistance Committee (RAC),

which is chaired by the Branch Chief of the DHS/FEMA Region VI Office. Formal approval of the RERPs was granted by FEMA on April 25, 1988 under 44 CFR 350.

The DHS/FEMA Region VI Office evaluated a drill on October 21 - 22, 2008, to assess the capabilities of local emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving the Waterford 3 Steam Electric Station. The purpose of this report is to present the results and findings on the performance of the offsite response organizations (OROs) during a simulated radiological emergency.

The findings presented in this report are based on the evaluations of the federal evaluation team, with final determinations made by the DHS/FEMA Region VI Office RAC Chair. The criteria utilized in the evaluation process are contained in:

NUREG-0654/FEMA-REP-1, Rev. 1, Criteria for Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants, November 1980; and Interim REP Program Manual, including the Radiological Emergency Preparedness Exercise Evaluation Methodology (August 2002).

Section 3 of this report, entitled "Drill Overview," presents basic information and data relevant to the drills. This section of the report contains a description of the Emergency Planning Zone (EPZ) and a listing of all participating jurisdictions and functional entities that were evaluated.

Section 4 of this report, entitled "Drill Evaluation and Results," presents detailed information on the demonstration of applicable evaluation areas at each jurisdiction or functional entity. If applicable, this section also contains: (1) descriptions of all Deficiencies and Areas Requiring Corrective Actions (ARCAs) assessed during the drill and recommended corrective actions and (2) descriptions of unresolved ARCAs assessed during previous exercises and the status of the OROs efforts to resolve them.

3. Drill Overview This section contains data and basic information relevant to the October 21 - 22, 2008, Reception Center and Medical drills to test the offsite response capabilities in the area surrounding the Waterford 3 Steam Electric Station. This section of the report includes a description of the Emergency Planning Zone and a listing of all participating jurisdictions and functional entities that were evaluated.

3.1. EPZ Description The area within 10 miles of Waterford 3 Steam Electric Station (Waterford 3) is entirely in the State of Louisiana. The most prominent natural feature in the Emergency Planning Zone (EPZ) is the Mississippi River running from west-northwest to east-southeast through the middle of the area. The Waterford 3 EPZ involves two parishes, St. John the Baptist Parish and St. Charles Parish.

There are several communities near the site within the 10-mile EPZ. These include Killona, Montz, Norco, Destrehan, Hahnville, Luling, LaPlace, Edgard, Reserve, and Garyville. The 2000 census estimated the population of the EPZ to be 91,116 persons mainly concentrated in towns along the Mississippi River. There are two hospitals, three nursing homes, and two incarceration facilities in the EPZ.

The major highways include I-10, I-310, I-55, U.S. Highways 61, 51, and 90, and Louisiana Highways 18 and 3127. There are four railways in the EPZ, which are the Canadian National Railroad, Kansas City Southern Railroad, Union Pacific Railroad, and Burlington Northern Railroad. The Waterford 3 EPZ is divided into 16 Protective Action Sections for the purpose of emergency response and implementation of protective actions.

ISSUE NO.: 70-08-6a1-A-01 CRITERION: Reception center/emergency worker facility has appropriate space, adequate resources, and trained personnel to provide monitoring, decontamination, and registration of evacuees and/or emergency workers.

CONDITION: The vehicle monitor moved too fast and held the probe too far from the vehicle surface to properly detect the presence of radiation.

Secondly, when the vehicle monitor identified contamination he did not respond appropriately by reporting the contamination levels and allowing the recorder to record the contamination levels on the appropriate forms.

POSSIBLE CAUSE: The vehicle monitor did not refer to the procedure for proper monitoring technique and reporting procedures. This is a new location and additional training on procedures and technique is recommended.

REFERENCE:

NUREG-0654, K.5.a (Decontamination and Monitoring of emergency personnel, supplies, instruments, and equipment and for waste disposal) and FEMA-REP-22 (Contamination Monitoring Guidance for Portable Instruments Used for Radiological Emergency Response to Nuclear Power Plant Accidents).

EFFECT: The monitor may not have detected contamination on the vehicle because he was using the improper monitoring technique. In addition, the monitor did not respond appropriately when contamination was detected on 8

the vehicle. This undetected and unreported contamination may result in a spread of contamination to other clean areas or persons.

CORRECTIVE ACTION DEMONSTRATED: The controller coached the monitor on the proper technique and the monitor was able to re-demonstrate the activity using effective monitoring techniques. The controller stopped play for the second time and coached the monitor on the proper procedure.

After coaching, the monitor was able to respond appropriately by informing the teams recorder of the contamination levels found and directing the vehicle driver to the impound area.

State Of Louisiana Waterford-3 SES - 2008 Evaluated Drill October 21 - 22, 2008 Objectives and Extent-of-Play (EOP) Agreement ARCAs None EVALUATION AREA 3: PROTECTIVE ACTION IMPLEMENTATION Sub-Element 3.a - Implementation of Emergency Worker Exposure Control Criterion 3.a.1: The OROs issue appropriate dosimetry and procedures, and manage radiological exposure to emergency workers in accordance with the plans and procedures. Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart.

State Of Louisiana Waterford-3 SES - 2008 Evaluated Drill October 21 - 22, 2008 Objectives and Extent-of-Play (EOP) Agreement EVALUATION AREA 6: SUPPORT OPERATIONS/FACILITIES Sub-Element 6.a - Monitoring and Decontamination of Evacuees and Emergency Workers, and Registration of Evacuees Criterion 6.a.1: The reception center/emergency worker facility has appropriate space, adequate resources and trained personnel to provide monitoring, decontamination and registration of evacuees and/or emergency workers.

Locations Pontchartrain Center Reception Center, Paulina Volunteer Fire Department Decon Station Extent of Play Both the Pontchartrain Center Reception Center staff and the Paulina Volunteer Fire Department Decon Station staff will be notified alerted and mobilized by their respective notification points. A roster indicating 24-hour coverage for their facilities will be available, and a shift change will not be demonstrated at either location. A portal monitor will be used to demonstrate evacuee personnel monitoring at the Pontchartrain Center Reception Center. Evacuee decontamination will be simulated. A decontamination area will actually be set-up for the demonstration.

Correction on the spot, at the discretion of and concurrence between the evaluator and the controller, may be acceptable at these locations.

Registration and assignment of evacuees will be demonstrated at the reception center. The Reception Center serves 4,592 evacuees (20% of 22,960 total response area population) and will process 6 simulated evacuees (1 simulated/contaminated) during the drill. The Reception Center is prepared to process 1,646 vehicles (20% of 8,231 vehicles) and will process 2 simulated evacuee vehicles (1 contaminated) during the drill.

State Of Louisiana Waterford-3 SES - 2008 Evaluated Drill October 21 - 22, 2008 Objectives and Extent-of-Play (EOP) Agreement Sub-Element 6.b - Monitoring and Decontamination of Emergency Worker Equipment Criterion 6.b.1: The facility/ORO has adequate procedures and resources for the accomplishment of monitoring and decontamination of emergency worker equipment including vehicles.

State Of Louisiana Waterford-3 SES - 2008 Evaluated Drill October 21 - 22, 2008 Objectives and Extent-of-Play (EOP) Agreement management, communications and arrangements for shelter, food services, family assistance, child care, medical care and sanitation. They will also be prepared to discuss transportation issues, screening for contamination at the congregate care center and evacuee tracking.

ARCAs None Sub-Element 6.d - Transportation and Treatment of Contaminated Injured Individuals Criterion 6.d.1: The facility/ORO has the appropriate space, adequate resources and trained personnel to provide transport, monitoring, decontamination and medical services to contaminated injured individuals.

Locations West Jefferson Air Care, West Jefferson Medical Center Extent of Play West Jefferson Medical Center will be contacted. A simulated contaminated and injured person will be identified and transportation will be provided by the West Jefferson Air Care helicopter. Medical transportation will be staged at the West Jefferson Medical Center at the beginning of the drill. If the ambulance is not available due an actual medical emergency, transportation to the hospital will occur using a utility vehicle.

Correction on the spot, at the discretion of and concurrence between evaluator and controller, may be acceptable at this location.

STATE OF LOUISIANA WATERFORD-3 SES October 21 & 22, 2008 I. Situation This drill will be conducted for the purpose of testing the ability of the following organizations to address an emergency at the Waterford 3 SES:

Paulina VFD Emergency Worker Decontamination Station, and the Ponchartrain Center Reception Center.

II. Summary of Events Tuesday, October 21, 2008 At approximately 18:00, the Drill Control Team will notify the St. James Parish EOC to notify the Emergency Worker Decontamination Station staff. A simulated emergency worker is directed to the Paulina VFD emergency worker decontamination station for evaluation. Upon arrival at the Paulina VFD station, the emergency workers car is found to be contaminated and is impounded. When he is monitored, the emergency worker is found to be contaminated and undergoes decontamination procedures. After these procedures are performed, the emergency worker is found to be free of contamination and is dismissed. Prior to the termination of the drill, a list of qualified replacement personnel will be made available. No shift change will take place.

Wednesday, October 22, 2008 At approximately 13:30, the Drill Control Team will notify the Jefferson Parish EOC to notify the Reception Center staff. Activation and staffing of the Reception Center will begin at this time. At approximately 14:00 a simulated evacuee arrives at the Ponchartrain Center Reception Center for evaluation. Upon arrival at the Ponchartrain Center Reception Center, the simulated evacuees car is found to be contaminated and is impounded. When the simulated evacuee is monitored, he is found to be contaminated and undergoes decontamination procedures. After these procedures are performed, the simulated evacuee is found to be free of contamination and is dismissed. In all, a total of four simulated evacuees and two simulated evacuee vehicles will undergo monitoring procedures.

Evacuee registration and shelter assignment will also take place at this time. Prior to the termination of the drill, a list of qualified replacement personnel will be made available. No shift change will take place.

INTRODUCTION In the interest of assuring the health and safety of the general public in the area of the Waterford 3 Steam Electric Station, in the event of a radiological emergency, Waterford 3 conducts drills jointly with the Federal, State and local agencies on a periodic basis.

This manual contains the scenario of activities and supporting data describing the Waterford 3 Emergency Preparedness Offsite Medical Drill.

This scenario was developed by postulating events that will require response by offsite medical assistance organizations.

During this drill, the support organizations will respond to the postulated events and conditions that have been selected to provide the level of activity necessary to meet the scenario objectives.

West Jefferson Medical Center Air Care Ambulance and West Jefferson Medical Center will participate in this drill.

Drill participants will have no prior knowledge of the sequence of events. The drill scenario will allow those individuals and agencies assigned to respond during a radiological medical emergency to demonstrate their performance according to current emergency preparedness plans and procedures.

The scenario is a mechanism by which selected drill controllers and monitors will initiate and evaluate the activities of the drill participants. The drill will be initiated at the Waterford 3 helicopter pad at 08:00 on October 22, 2008 and is expected to last approximately three hours.

Participating organizations are shown in Section III, Guidelines, of this manual.

A. WEST JEFFERSON MEDICAL CENTER

1. Demonstrate the adequacy of the equipment, procedures, supplies and personnel of medical facilities responsible for treatment of contaminated, injured, or exposed individuals.

B. WEST JEFFERSON MEDICAL CENTER AIR CARE AMBULANCE

1. Demonstrate the adequacy of vehicles, equipment, procedures and personnel for transporting contaminated, injured or exposed individuals.

1. The offsite medical response drill will be conducted on October 22, 2008.
2. Participation is limited to the West Jefferson Medical Center Air Care Ambulance staff, West Jefferson Medical Center Emergency Room Staff and the Drill Control Team. The Waterford 3 plant staff WILL NOT participate.
3. Communications between the Waterford 3 plant and West Jefferson Medical Center are simulated by the Drill Control Team.
4. A simulated contaminated injured person and a Health Physics technician are staged at the Waterford 3 helicopter pad at the beginning of the drill.
5. The drill is initiated by the Drill Control Team requesting the dispatch of the West Jefferson Medical Center Air Care Ambulance to transport the patient to West Jefferson Medical Center. The Drill Control Team then contacts West Jefferson Medical Center simulating a report of a medical emergency.
6. If West Jefferson Medical Center Air Care Ambulance is unable to support to this drill, then St.

Charles Hospital Ambulance Service will transport the patient to West Jefferson Medical Center.

7. If St. Charles Hospital Ambulance Service is unable to support to this drill, transportation of the patient to West Jefferson Medical Center is simulated with a company vehicle.
8. All communications are preceded and ended with "THIS IS A DRILL."
9. The simulated medical emergency continues until the patient is decontaminated and treated by the West Jefferson Medical Center medical staff.
10. A critique is conducted immediately following the termination of the drill.

SUMMARY

At approximately 07:45 AM, a Mechanical Maintenance Technician at Waterford 3, who has been repairing a leak on the A Low Pressure Safety Injection Pump, slips and falls down breaking his right forearm and striking his forehead on a pump support. The Drill Control Team simulates the Control Room response and UNT-007-018, First Aid and Medical Care, is implemented. Response by the Emergency First Aid Team is simulated and primary surveys of the patient are performed.

At 08:00, the Drill Control Team calls the West Jefferson Medical Center Air Care Ambulance Service to request transportation of the patient to West Jefferson Medical Center. After requesting the ambulance, the Drill Control Team calls the West Jefferson Medical Center to report the simulated medical emergency. The patient's primary survey information (vital signs) is provided.

Upon arrival at West Jefferson Medical Center, the patient is taken into the REA, decontaminated and treated for the simulated injuries. After the treatment has proceeded to the point that the patient would be admitted to the hospital and the ambulance has been surveyed and released, the medical drill is terminated and a critique of the events is conducted.

1. Paulina-Grand Point-Belmont Volunteer Fire Department ISSUE NO.: 70-08-6a1-P-02 CONDITION: Portions of the Checklist which make up a part of the plan are out of date or misleading. For example, the Personnel Contamination Report (Section IV Form 2 of Checklist 3) shows a CD V-700 as the monitoring instrument rather than the Ludlum 3-A which was the instrument demonstrated. Also,Section III of Checklist 3 discusses a portal monitor.

This devise is not shown in other parts of the Checklist and was not demonstrated.
