ATTACHMENT 1 L-08-320 Technical Specification 5.6.6.2.1 Steam Generator Tube Inspection Report Page 1 of 8 The following information satisfies the reporting requirement of Beaver Valley Power Station Technical Specification (TS) 5.6.6.2.1. Refer to the tables on Page 7 of this attachment which define abbreviations, codes and other terms used throughout this report.
Information Required by Technical Specification 5.6.6.2.1 Technical Specification 5.6.6.2.1 requires reporting of the following information after completion of an inspection performed in accordance with the Specification 5.5.5.2, Unit 2 Steam Generator (SG) Program. Information required by TS 5.6.6.2.1.a through TS 5.6.6.2.1.i is provided below for activities performed during the spring 2008 maintenance and refueling outage (2R13).
- a. The scope of inspections performed on each steam generator Primary side inspection scope involved the following:
- 100% full length bobbin inspection (except Row 1 and 2 U-bends) per Generic Letter 95-05
- 100% Plus Point inspection of Row 1 and 2 U-bends
- 100% Plus Point inspection of tube support plate DSI/DNI indications
- 100% Plus Point inspection of tube support plate mix residual signals < 550 and >
1.25 volts
- 100% Plus Point inspection of tube support plate mix residual signals > 2.0 volts
- 100% Plus Point inspection of the hot leg top of tubesheet from 6 inches above to 3 inches below the top of tubesheet
- 100% Plus Point inspection of dented hot leg tube support plate's > 5.0 volts
- 100% Plus Point inspection of dents > 2.0 but < 5.0 volts at the 1st, 2nd, 3rd and 4th hot leg tube support plate's
- 100% Plus Point inspection of dents located at anti-vibration bar (AVB) sites (+/- 1 inch of AVB)
- 100% Plus Point inspection of freespan [tubesheet hot leg (TSH) to tubesheet cold leg (TSC)] dings > 5 volts
- 100% Plus Point inspection (High Frequency) of Row 1 U-bends with excessive noise values
- 100% Plus Point inspection of bulge and over expansion signals in the hot leg tubesheet below the F* distance but above the tubesheet neutral axis L-08-320 Page 2 of 8
- 100% Plus Point inspection of bobbin "I-codes", such as freespan differential signals meeting the change criteria (Phase rotation: > 100 and/or Voltage amplitude: > 0.5 volts)
- 100% Plus Point inspection of freespan signals not resolved as manufacturing burnish marks or without a historical review that determined the signal was not degradation related
- 100% Plus Point inspection of U-bend primary water stress corrosion cracking indications reported with the mid-range Plus Point coil
- 25% Plus Point inspection of tube support plate mix residual signals > 1.5 but < 2.0 volts
- 25% Plus Point inspection of Row 3 through 8 U-bends
- 25% Plus Point inspection of freespan paired dings (Dings located - 1800 apart)
NOTE: The initial freespan ding inspection program was expanded in all SGs due to the detection of a single circumferential Outside Diameter Stress Corrosion Cracking indication within a freespan ding in Steam Generator "C".
The inspection program scope for Steam Generator "C" was increased to include:
(a) 100% of all dings < 5.0 volts from the 5 th hot leg tube support plate to 4 inches above the 8 th hot leg tube support plate, (b) 100% of all dings < 5.0 volts from the 5 th cold leg tube support plate to 4 inches above the 8 th cold leg tube support plate, (c) 25% of all dings < 5.0 volts from the tubesheet - hot leg (secondary face) to the 5 th hot leg tube support plate, and (d) 25% of all dings < 5.0 volts from the tubesheet - cold leg (secondary face) to the 5 th cold leg tube support plate.
For Steam Generator "A" and Steam Generator "B", the scope expansion included:
(a) 25% of all dings < 5.0 volts from the tubesheet - hot leg (secondary face) to 4 inches above the 8 th hot leg tube support plate, and (b) 25% of all dings < 5.0 volts from the tubesheet - cold leg (secondary face) to 4 inches above the 8 th cold leg tube support plate.
No additional indications were reported from this scope expansion. It should be noted, that this expanded scope exceeded the recommendations of the EPRI PWR Examination Guidelines, Revision 6.
- Tube plug visual inspections (As-found and As-left conditions)
Secondary Side:
- Visual inspection of possible loose part locations reported from the 2R1 3 eddy current examinations L-08-320 Page 3 of 8
- Visual inspection of the annulus and tube lane regions
" Foreign object search and retrieval (FOSAR), as necessary
" Visual inspections of Steam Generator "A" moisture separation equipment, feedwater header and selected J nozzles. UT thickness measurements of selected areas of the feedwater header.
- b. Active degradation mechanisms found The following active degradation mechanisms were observed during the 2R13 inspections:
" Axial outside diameter stress corrosion cracking at non-dented tube support plate intersection
" Axial outside diameter stress corrosion cracking within the hot leg sludge pile
" Axial outside diameter stress corrosion cracking at the hot leg top of tubesheet expansion transitions
" Axial primary water stress corrosion cracking at the hot leg top of tubesheet expansion transitions
" Circumferential outside diameter stress corrosion cracking at the hot leg top of tubesheet expansion transitions
- Circumferential outside diameter stress corrosion cracking at freespan dings
" Wear at anti-vibration bar sites
- c. Nondestructive examination techniques utilized for each degradation mechanism For the degradation mechanisms observed during 2R13, bobbin coil probes were used as the primary method of detection except in areas (e.g. at or below top of tubesheet, low row U-bends) where bobbin probes are not qualified. In those areas, Plus Point Probes were employed. Plus Point Probes were also used to re-examine any ambiguous bobbin coil signals that were reported.
The 2R13 examination techniques were qualified, for detection and sizing (as applicable) of the existing and potential degradation mechanisms listed in the 2R13 degradation assessment, in accordance with Appendix H of the EPRI PWR SG Examination Guidelines, Revision 6. These techniques are tabulated below.

(ETSS) is listed for In accordance Appendix H of the EPRI PWR SG Examination Guidelines, Revision 6, site-specific examination technique sheets were developed for each existing and potential degradation mechanism identified in the 2R13 degradation assessment.
The examination techniques utilized during 2R1 3 are considered "Site Validated" as defined in the EPRI PWR SG Examination Guidelines, Revision 6, for each existing and potential degradation mechanism listed in the 2R13 degradation assessment.
- d. Location, orientation (if linear), and measured sizes (if available) of service-induced indications lists information that includes the location, orientation and measured sizes of service induced indications. Further explanation of the information provided in Attachment 2 is given below:
- 1) Indications reported from the bobbin probe listed as "I-codes" (e.g. DNI, DSI, DTI or NQI) were re-inspected with the Plus Point probe. If the Plus Point probe indicated that no degradation or flaws were present, the tube remained in-service. If the Plus Point probe indicated that degradation or flaws were present, the tube was removed from service via plugging.

- 2) Indications reported from the bobbin probe listed as possible tube support plate indications (PSI) were re-inspected with the Plus Point probe. If the Plus Point probe indicated a possible tube support plate ligament breach existed (CSI), it was further evaluated by the original equipment manufacturer engineering group.
All ligament breach extents were found to be acceptable for continued plant operation.
- 3) New anti-vibration bar indications reported from the bobbin probe listed as percent through wall PCT were re-inspected with the Plus Point probe. If the Plus Point probe indicated no flaws were present within the wear scar, a wear call (WAR) was assigned to the Plus Point report. No tubes were removed from service for percentages of wall degradation exceeding the Technical Specification limit of 40 percent.
- 4) Indications reported from the Plus Point probe listed as "I-codes" (e.g. SAI, SCI or MCI) were removed from service via plugging. All circumferential indications were stabilized prior to plug installation.
- e. Number of tubes plugged or repaired during the inspection outage for each active degradation mechanism The following tables list the number of tubes plugged for each active degradation mechanism during 2R13. To date, no tubes have been repaired (sleeved).

- f. Total number and percentage of tubes plugged or repaired to date The total number of tubes plugged as of the end of 2R1 3 is 549 (5.42%). To date, no tubes have been repaired (sleeved).
- g. The results of condition monitoring, including the results of tube pulls and in-situ testing During the 2R13 steam generator tube inspections, no indications were observed that exceeded the structural integrity limits for either axial or circumferential degradation (i.e., burst integrity greater than three times normal operating primary to secondary pressure differential). Therefore, no tubes contained indications that could potentially challenge the tube integrity recommendations of NEI 97-06, Revision 2, "Steam Generator Program Guidelines." The relative severity levels of the degradation observed during 2R1 3 was judged consistent with or bounded by the levels associated with the spring 1999 (2R07) through fall 2006 (2R12) refueling outage inspection results.
Based on the indications observed at 2R13, the steam generator tubing is expected to meet all operational structural and leakage integrity requirements at the fall 2009 refueling outage (2R14). None of the indications reported during 2R13 represented a leakage potential at postulated main steam line break conditions.
As a conservative measure, the single circumferential outside diameter stress corrosion cracking indication located within a free span ding in Steam Generator "C" was in-situ tested even though the indication did not meet the established in-situ screening criteria. No evidence of leakage was reported at any of the target pressures.
Based on the degradation observed during 2R13, no tube pulls were required.

- h. The effective plugging percentage for all plugging and tube repairs in each steam generator The following table lists the plugging percentages for each steam generator as of the end of 2R13. To date, since no tube sleeves have been installed, the effective plugging percentage is not applicable.
