SEQUOYAH NUCLEAR PLANT -NATIONAL POLLUTANT DISCHARGE ELIMINATION SYSTEM (NPDES) COMPLIANCE EVALUATION INSPECTION (CEI)

On July 8-9, 2008, during the Tennessee Department of Environment and Conservation (TDEC) National Pollutant Discharge Elimination System (NPDES) Compliance Evaluation Inspection (CEI), comments and recommendations were discussed and SQN initiated the following Problem Evaluation Reports (PERs). These issues will be corrected and tracked to completion through TVA's corrective action program.

PER 148118 During the Tennessee Department of Environment and Conservation (TDEC) Compliance Evaluation Inspection (CEI) on July 8-9, 2008 a recommendation was made to set up a periodic frequency to clean out all SONs storm water outfalls of any debris, trash, etc...that might be blocking the discharge piping. Some trash andc debris was observed at storm water outfalls #7,

  1. 8, and #9.

PER 148119 During the Tennessee Department of Environment and Conservation (TDEC) Compliance Evaluation Inspection (CEI) on July 8-9, 2008 an observation was made that the boom in the Cooling Tower Basin close to Gate Structure #2 was not fully extended across the water.

There was approximately a 4-5 foot gap between the boom and the bank. Also, a recommendation was made to install a second boom behind the first boom as a best management practice (BMP). The boom is in the CT basin during H-150M (molluscicide) treatments because the H-1 50M treatments have been observed to produce small amounts of foam. Some biological scum/film was observed on the surface of the CT basin but nothing was being discharged to the TN River. SQNs NPDES permit (TN0026450) states, "There shall be no distinctly visible floating scum, solids, oil sheen, visible foam, and other floating matter discharged with the wastewaterto the receiving stream (TN River)."

PER 148121 During the Tennessee Department of Environment and Conservation (TDEC) Compliance Evaluation Inspection (CEI) on July 8-9, 2008 an observation was made that NPDES sample Chain of Custody (COC) forms are not being filled out completely. SQN as the sampler and Environmental Sciences Corp as the receiving lab are filling the COCs out completely and correctly. The deficiency is when the samples are received and relinquished by the TVA mail room.

PER 148122 During the Tennessee Department of Environment and Conservation (TDEC) Compliance Evaluation Inspection (CEI) on July 8-9, 2008 an observation was made that 0-PI-CEM-000-460.3 referenced the wrong approved inorganic test procedure. The PI references EPA methods 330.4 and 330.5. Those methods were removed from the list of approved inorganic test procedures (40CFR136.3). This is only a reference problem and not a problem with the test procedure currently used. The test procedure currently used is an approved test procedure.
