The letter is from an insurance company for Davis-Besse and discusses the vessel head wastage at Davis-Besse and includes a discussion about the adequacy of the conclusion in the Root Cause Report and other documents associated with that issue. Particular information in the attachment indicates, "the development of the large crack at Davis-Besse Nozzle 3 and the subsequent wastage cavity development in a much shorter time frame that the root cause report concluded ......

To all, A call will be held with FENOC (Davis Besse Licensee) today at 11:00 EST to discuss a letter from their insurer alleging a safety-concern based upon work by "Eponent Failure Analysis Associates and Altran Solutions Corporation" which concluded that the Davis Besse head wastage cavity could have formed in just a few weeks. Further, this report alleges both crack growth rates and head material erosion rates are "significantly higher than previously assumed." Because of the generic ramifications we would like to have appropriate NRR & RES staff participate. We have not seen this report, but I have attached the insurer's letter.

Mel, Given the generic implications, it might be a good idea to give NRR the chance to listen in as well. Might save us a lot of trouble later passing on the info to them. I think Terrance and Matt are both out of the office today. Could you please see if there is someone else from the appropriate branch who could participate. I think John Sow (sp?) was mentioned as an acting there.

Eric, Just got a call from the licensee that 1100 EST, 1000 CST looks good for a phone call on the NEIL letter.

Tentatively would be several people from the DB site and possibly 1 or 2 people from the company that produced the report for FE.
